摘  要:目的自噬相关蛋白Atg14在碘难治性分化型甲状腺癌(RAIR-DTC)中的作用报道较少。文中旨在探讨自噬相关蛋白Atg14在RAIR-DTC的诊断价值。方法回顾性收集2013年1月至2019年12月广西医科大学第一附属医院核医学科72例行甲状腺癌手术治疗的DTC患者。根据治疗效果分为RAIR-DTC组(碘难治患者,n=37)和NRAIR-DTC组(非碘难治患者,n=35)。采用免疫组化方法检测癌组织中Atg14的表达,并分析其表达量与患者血清刺激性甲状腺球蛋白(sTg)、影像学T/B及临床病理特征的关系。结果^(131)I治疗前,RAIR-DTC组癌组织中Atg14相对表达量(0.079±0.020)较NRAIR-DTC组(0.034±0.006)明显升高(P=0.001)。ROC曲线分析结果显示:Atg14的AUC为0.995,诊断阈值为0.047,灵敏性为94.6%,特异性为100%,95%CI:0.987-1.000(P<0.01)。两次^(131)I治疗前,与NRAIR-DTC组血清学sTg水平相比,RAIR-DTC组更高(P<0.05)。RAIR-DTC组sTg同比下降率分别为(0.114±0.759)和(0.900±0.191),明显低于NRAIR-DTC组(P<0.05)。两次^(131)I治疗后,RAIR-DTC组T1/T2均高于NRAIR-DTC组(P<0.05)。结论Atg14在RAIR-DTC癌组织中高表达,其可能成为RAIR-DTC的诊断标志物之一。Objective The role of autophagy-related protein Atg14 in iodine refractory differentiated thyroid carcinoma(RAIR-DTC)is rarely reported.This review aims to investigate the diagnostic value of autophagy-related protein Atg14 in RAIR-DTC.Methods A total of 72 DTC patients who underwent surgical treatment for thyroid cancer in the Department of Nuclear Medicine,the First Affiliated Hospital of Guangxi Medical University from January 2013 to December 2019 were retrospectively collected.According to the treatment effect,the patients were divided into RAIR-DTC group(iodine-refractory patients,n=37)and NRAIR-DTC group(non-iodine-refractory patients,n=35).The expression of Atg14 in cancer tissues was detected by immunohistochemistry,and its relationship with serum stimulant thyroglobulin(sTg),imaging T/B and clinicopathological features was analyzed.Results Before I-131 treatment,the relative expression of Atg14 in RAIR-DTC group(0.079±0.020)was significantly higher than that in NRAIR-DTC group(0.034±0.006)(P=0.001).ROC curve analysis showed that the AUC of Atg14 was 0.995,the diagnostic threshold was 0.047,the sensitivity was 94.6%,the specificity was 100%,95%CI:0.987-1.000(P<0.01).Compared with NRAIR-DTC group,the level of serological sTg in RAIR-DTC group was higher before two I-131 treatments(P<0.05).The decrease rates of sTg in RAIR-DTC group were(0.114±0.759)and(0.900±0.191),which were significantly lower than those in NRAIR-DTC group(P<0.05).After two I-131 treatments,the levels T1/T2 in RAIR-DTC group were higher than those in NRAIR-DTC group(P<0.05).Conclusion Atg14 is highly expressed in RAIR-DTC cancer tissues,which may become one of the diagnostic markers of RAIR-DTC.
