摘 要:目的 探讨富自体浓缩生长因子(CGF)结合Bio-Oss骨粉在颌骨囊肿患者术后骨缺损区的修复效果。方法 将77例行囊肿刮治术的颌骨囊肿患者随机分为观察组(38例)和对照组(39例);其中,对照组术后予以Bio-Oss骨粉修复骨缺损区,观察组术后予以CGF结合Bio-Oss骨粉修复骨缺损区。记录术前及术后即刻、1个月、3个月时骨缺损填充区域CT值,测量术后3、6个月时垂直骨和舌侧骨吸收量、骨密度、囊肿体积和面积减小量、上颌骨和下颌骨高度以及颌骨宽度,于术后6个月评估临床效果,并观察术后并发症发生情况。结果 与术前相比,两组术后即刻、1个月、3个月时骨缺损填充区域CT值均增加(P<0.05),且观察组增加更加明显(P<0.05)。两组术后6个月时垂直骨和舌侧骨吸收量、骨密度、囊肿体积和面积减小量、上颌骨和下颌骨高度以及颌骨宽度均优于术后3个月(P<0.05);观察组术后3、6个月时垂直骨和舌侧骨吸收量低于对照组(P<0.05),而骨密度、囊肿体积和面积减小量、上颌骨和下颌骨高度以及颌骨宽度高于对照组(P<0.05)。观察组修复总有效率高于对照组(92.11%vs.76.92%)(P<0.05),而术后并发症总发生率低于对照组(21.05%vs.35.90%)(P<0.05)。结论 CGF结合Bio-Oss用于骨粉在颌骨囊肿患者术后骨缺损区的修复效果较好,能减少骨吸收,促进骨愈合,提高骨密度,恢复颌骨外形,且并发症少。Objective To investigate the repairing effect of autologous concentrated growth factor(CGF) combined with Bio-Oss bone powder on the bone defect in the patients with jaw cyst after surgery.Methods A total of 77 patients undergoing jaw cyst curettage were randomly divided into two groups of A(38 cases) and B(39 cases).The patients in group A accepted Bio-Oss bone powder, while which in group B received CGF combined with Bio-Oss bone powder for repairing the bone defect after surgery.The CT value in the bone defect area was recorded before surgery, immediately after surgery and in the 1and 3month after surgery.The vertical bone and labial bone resorption, bone mineral density, decrease of cyst volume and area, height of maxilla and mandible, and width of jaw were measured in the 3and 6month after surgery.The clinical effect was evaluated after 6 months, and the postoperative complications were observed.Results Compared with before, the CT values in the bone defect area were increased immediately after surgery and in the 1and 3month after surgery in both groups(P<0.05),which were higher in group B than those in group A(P<0.05).The vertical bone and labial bone resorption, bone mineral density, decrease of cyst volume and area, height of maxilla and mandible, and width of jaw were better in the 6month after surgery than those in the 3month after surgery in both groups(P<0.05).The vertical bone and labial bone resorption were lower, while the bone mineral density, decrease of cyst volume and area, height of maxilla and mandible, and width of jaw were higher in group B than those in group A in the 3and 6month after surgery(P<0.05).The total effectiveness rate of repairing in group B was higher than that in group A(92.11% vs.76.92%)(P<0.05),while the incidence of postoperative complications in group B was lower than that in group A(21.05% vs.35.90%)(P<0.05).Conclusion CGF combined with Bio-Oss bone powder is effective in the repair of bone defect in the patients with jaw cyst after surgery with less bone resorption, qui
