周边虹膜切除术治疗高眼压状态下原发性急性闭角型青光眼的疗效研究

摘　　要：目的研究周边虹膜切除术治疗高眼压状态下原发性急性闭角型青光眼的临床疗效。方法120例高眼压状态下原发性急性闭角型青光眼患者,采用随机抽签的方式分为实验组和对照组,每组60例。对照组选择小梁切除术治疗,实验组选择周边虹膜切除术治疗。比较两组患者并发症发生情况、视力及眼压恢复情况、治疗效果。结果实验组并发症发生率11.67%低于对照组的30.00%,差异具有统计学意义(P<0.05)。实验组患者视力为(0.56±0.11)、眼压为(12.24±2.51)mm Hg(1 mm Hg=0.133 kPa),均优于对照组的(0.41±0.07)、(16.57±1.16)mm Hg,差异均具有统计学意义(P<0.05)。实验组治疗总有效率90.00%高于对照组的75.00%,差异具有统计学意义(P<0.05)。结论对高眼压状态下原发性急性闭角型青光眼患者进行周边虹膜切除术治疗,有利于减少并发症产生、恢复眼压及视力,患者病情得到有效改善,值得在临床推广。Objective To investigate the clinical efficacy of peripheral iridectomy in the treatment of primary acute angle closure glaucoma under high intraocular pressure.Methods A total of 120 patients with primary acute angle-closure glaucoma under high intraocular pressure were randomly divided into experimental group and control group,with 60 cases in each group.The control group was treated with trabeculectomy,while the experimental group was treated with peripheral iridectomy.The incidence of complications,visual acuity and intraocular pressure recovery,and therapeutic effect were compared between the two groups.Results The incidence of complications 11.67%in the experimental group was lower than 30.00%in the control group,and the difference was statistically significant(P<0.05).The visual acuity in the experimental group was(0.56±0.11),and the intraocular pressure was(12.24±2.51)mm Hg(1 mm Hg=0.133 kPa),which were better than(0.41±0.07)and(16.57±1.16)mm Hg in the control group,and the differences were statistically significant(P<0.05).The total effective rate 90.00%of the experimental group was higher than 75.00%of the control group,and the difference was statistically significant(P<0.05).Conclusion Peripheral iridectomy in patients with primary acute angle-closure glaucoma under high intraocular pressure can reduce the occurrence of complications,recover the intraocular pressure and visual acuity,and effectively improve the patient's condition,which is worthy of clinical promotion.
