疏肝解郁汤联合多潘立酮对功能性消化不良患者胃肠激素分泌的影响

摘　　要：目的探讨疏肝解郁汤联合多潘立酮对功能性消化不良(Functional dyspepsia,FD)患者胃肠激素分泌的影响。方法选取2019年1月—2021年1月期间河南省商丘市第一人民医院收治的FD患者90例,采用随机摸球法分为对照组和联合组,每组各45例。对照组给予多潘立酮治疗,在此基础上联合组加用疏肝解郁汤治疗,两组用药时间为4周。观察比较两组患者治疗前后中医证候症状(胃脘疼痛、胃脘部胀满不适、嗳气反酸、纳差食少、口苦口干、失眠多梦、头身困重、烦躁易怒、排便不爽)积分、血清胃肠激素[胃动素(Motilin,MTL)、生长抑素(Somatostatin,SS)、胃泌素(Gastrin,GAS)]水平变化、不良反应发生情况及临床疗效。结果治疗后联合组总有效率95.56%(43/45)与对照组82.22%(37/45)比较,差异有统计学意义(P<0.05)。治疗后两组患者中医证候积分较治疗前明显降低,差异有统计学意义(P<0.05);且联合组中医证候积分较对照组明显降低,差异有统计学意义(P<0.05)。治疗后两组患者MTL水平较治疗前明显升高,SS、GAS水平较治疗前明显降低,差异有统计学意义(P<0.05);且联合组MTL水平较对照组明显升高,SS、GAS水平较对照组明显降低,差异有统计学意义(P<0.05)。治疗期间,两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论疏肝解郁汤联合多潘立酮治疗FD的效果显著,能明显改善患者临床症状,降低胃肠激素分泌,且安全可靠。Objective To investigate the effects of Shugan Jieyu Decoction combined with domperidone on gastrointestinal hormone secretion in patients with functional dyspepsia(FD).Methods Ninty FD patients admitted to Shangqiu First People′s Hospital from January 2019 to January 2021 were randomly divided into a control group and a combined group by touch ball method,with 45 cases in each group.The control group was treated with domperidone,and the combined group was additionally given Shugan Jieyu Decoction for 4 weeks.The traditional Chinese medicine(TCM)syndrome symptom score(gastrointestinal pain,fullness and discomfort in the stomach,belching and acid reflux,poor appetite,bitter and dry mouth,insomnia and dreaminess,head and body heaviness,irritability,and bowel problems),the levels of serum gastrointestinal hormones such as motilin(MTL),somatostatin(SS),and gastrin(GAS),the incidence of adverse reactions,and the clinical effect were compared between the two groups.Results After treatment,the total effective rate of the combined group(95.56%)was significantly higher than that of control group(82.22%)(P<0.05).After treatment,the TCM syndrome symptom scores in the two groups were reduced(P<0.05),and the TCM syndrome symptom score in the observation group was lower than that in the control group(P<0.05).The level of MTL in the two groups was increased after treatment,and the levels of SS and GAS were decreased(P<0.05).The level of MTL in the combined group was higher than that in the control group,and the levels of SS and GAS were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Shugan Jieyu Decoction combined with domperidone has a significant effect on the treatment of FD,which significantly improves the clinical symptoms of patients and reduces the gastrointestinal hormone secretion with safety and reliability.
