黄连汤加减联合牙周基础治疗对重度慢性牙周炎患者血清炎症因子水平的影响

摘　　要：目的探讨黄连汤加减联合牙周基础治疗对重度慢性牙周炎患者血清炎症因子水平的影响。方法选取2018年12月—2020年8月期间于青岛市市立医院口腔科诊治的130例重度慢性牙周炎患者为研究对象,按随机数字表法分为研究组和对照组,每组各65例。对照组给予牙周基础治疗,观察组给予黄连汤加减联合牙周基础治疗。治疗1个月后,观察比较两组患者临床疗效,对比治疗前后两组患者中医证候积分、牙周指数[牙龈出血指数(Bleeding index,BI)、菌斑指数(Plaque index,PLI)、附着丧失(AttachmentLoss,AL)、牙周探诊深度(Periodontal probing depth,PD)]及血清炎症因子[白细胞介素-1β(Interleukin-1β,IL-1β)、白细胞介素-6(Interleukin-6,IL-6)、白细胞介素-23(Interleukin-23,IL-23)、肿瘤坏死因子-α(Tumor necrosis factor-alpha,TNF-α)]水平。结果治疗后观察组总有效率为98.46%(64/65)明显高于对照组83.08%(54/65),差异有统计学意义(P<0.05)。治疗后两组患者牙龈红肿、口臭、牙龈伴少量脓血性分泌物渗出、轻度疼痛、烦渴、喜冷饮、呕吐酸苦评分均较治疗前降低,差异有统计学意义(P<0.05);且研究组牙龈红肿、口臭、牙龈伴少量脓血性分泌物渗出、轻度疼痛、烦渴、喜冷饮、呕吐酸苦评分均较对照组明显降低,差异有统计学意义(P<0.05)。治疗后两组患者PLI、BI、AL、PD指数均较治疗前降低,差异有统计学意义(P<0.05);且研究组PLI、BI、AL、PD指数均较对照组明显降低,差异有统计学意义(P<0.05)。治疗后两组患者IL-1β、IL-6、IL-23、TNF-α水平均较治疗前降低,差异有统计学意义(P<0.05);且研究组IL-1β、IL-6、IL-23、TNF-α水平均较对照组明显降低,差异有统计学意义(P<0.05)。结论黄连汤加减联合牙周基础治疗能够有效降低重度慢性牙周炎患者血清IL-1β、IL-6、IL-23、TNF-α水平,效果显著,值得临床推广使用。Objective To explore the effect of Huanglian Decoction combined with basic periodontal treatment on serum inflammatory factors in patients with severe chronic periodontitis.Methods A total of 130 patients with severe chronic periodontitis diagnosed and treated in the Department of Stomatology of Qingdao Municipal Hospital from December 2018 to August 2020 were selected as the research subjects.According to the random number table,the patients were divided into observation group(65 cases,treated with Huanglian Decoction combined with basic periodontal treatment)and control group(65 cases,treated with basic periodontal treatment).The clinical curative effect of the two groups was compared after 1 month of treatment.In addition,the TCM syndrome score,periodontal indexes[gingival bleeding index(BI),plaque index(PLI),attachment loss(AL),periodontal probing depth(PD)]and the levels of serum inflammatory factors[interleukin-1β(IL-1β),interleukin-6(IL-6)and interleukin-23(IL-23),tumor necrosis factor alpha(TNF-α)]were compared between the two groups before and after treatment.Results After treatment,the total effective rate in the observation group was 98.46%(64/65),which was significantly higher than 83.08%(54/65)in the control group(P<0.05).The scores of gingival swelling,halitosis,gingiva with a small amount of pus and blood discharge,mild pain,polydipsia,desire for cold drink and vomiting in the two groups were lower than those before treatment(P<0.05),with the observation group lowering more obviously than the control group(P<0.05).The PLI,BI,AL and PD in the two groups were decreased compared with those before treatment(P<0.05),with the observation group having more obvious decrease than the control group(P<0.05).The levels of IL-1β,IL-6,IL-23 and TNF-preasthe two groups were reduced compared with those before treatment(P<0.05),with the observation group having more obvious reduction than the control group(P<0.05).Conclusion Huanglian Decoction combined with basic periodontal treatment could reduce the levels o
