摘 要:目的研究缺氧诱导因子-1α(Hypoxia inducible factor-1α,HIF-1α)和血管内皮生长因子(Vascular endothelial growth factor,VEGF)在食管鳞癌(Esophageal squamous cell carcinoma,ESCC)组织中的表达及临床意义。方法收集2019年9月-2019年12月就诊于新疆医科大学附属肿瘤医院胸外科诊断为食管鳞癌的40例患者的癌组织及癌旁组织。采用实时荧光定量聚合酶联反应检测HIF-1αmRNA和VEGF mRNA的表达水平,分析两者表达水平与患者临床病理学特征之间的关系,通过ROC曲线下面积评价两者诊断食管鳞癌的效能。结果与癌旁正常组织比较,食管鳞癌组织中HIF-1αmRNA和VEGF mRNA相对表达水平升高,差异有统计学意义(P<0.05)。HIF-1α高表达与肿瘤大小、分化程度、脉管癌栓、淋巴结转移和TNM分期有关(P<0.05);VEGF高表达与脉管癌栓、淋巴结转移和TNM分期密切相关(P<0.05)。经Pearson相关性分析HIF-1α和VEGF在食管鳞癌组织中的表达呈正相关(r=0.747,P<0.01)。ROC曲线分析显示,HIF-1αmRNA诊断食管鳞癌的曲线下面积为0.7744(95%CI:0.6700~0.8788),VEGF mRNA曲线下面积为0.9288(95%CI:0.8776~0.9799)。结论HIF-1αmRNA和VEGF mRNA在食管鳞癌组织中的表达水平升高,两者异常表达在促进食管鳞癌侵袭、转移方面可能存在协同作用。Objective To investigate the expression and clinical significance of hypoxia-inducible factor-1α(HIF-1α)and vascular endothelial growth factor(VEGF)in esophageal squamous cell carcinoma(ESCC).Methods The carcinoma tissues and adjacent tissues were collected from 40 patients and diagnosed in the hospital from September 2019 to December 2019.The expression levels of HIF-1αmRNA and VEGF mRNA were measured by quantitative real-time polymerase combination reaction,and the relationship between the two expression levels and the patient clinicopathological characteristics were analyzed,and the efficacy of the both oesophageal squamous cell carcinoma wasevaluated by the area under theROCcurve.ResultsCompared with adjacent normal tissues,the relative expressionlevels of HIF-1αmRNA and VEGF mRNA in esophageal squamous cell carcinoma tissues were increased,and thedifferences were statistically significant(P<0.05).HIF-1αexpression was associated with tumor size,differentia-tion,vascular tumor thrombus,lymph node metastasis and TNM stage(P<0.05);the expression of VEGF wasclosely related to vascular tumor thrombus,lymph node metastasis and TNM stage(P<0.05).Pearson correlationanalysis showed that HIF-1αand VEGF expression were positively correlated in esophageal squamous cell carcinoma(r=0.747,P<0.01).ROCcurve analysis showed that the area under the curve of HIF-1αmRNA diagnosis of e-sophageal squamous cell carcinoma was 0.7744(95%CI:0.6700~0.8788);the area under the curve of VEGFmRNA was 0.9288(95%CI:0.8776~0.9799).ConclusionThe expression levels of HIF-1αmRNA andVEGF mRNA are increased in esophageal squamous cell carcinoma,and the high levels play a synergistic role inpromoting the invasion and metastasis of esophageal squamous cell carcinoma.
