摘　　要：目的:探讨托吡酯联合丙戊酸钠对癫痫患者认知功能及血清炎症因子等的影响。方法:选取2020年1月-2021年1月武穴市中医医院收治的108例癫痫患者,采用随机数字表法将其分为观察组(n=54)及对照组(n=54)。对照组行丙戊酸钠治疗,观察组在对照组基础上加用托吡酯,对比两组的认知功能评分、血清炎症因子指标、神经营养指标及事件相关诱发电位P300波相关数值。结果:治疗6个月后,观察组的脑源性神经营养因子(BDNF)、神经生长因子(NGF)、事件相关诱发电位P300波幅均高于对照组,肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、中枢神经特异性蛋白(S100β)、事件相关诱发电位P300波的反应时间及潜伏期均低于对照组,蒙特利尔认知量表(MoCA)评分高于对照组,差异均有统计学意义(P<0.05)。结论:托吡酯联合丙戊酸钠治疗癫痫可改善患者的认知功能,降低炎症反应,改善神经营养状态,效果显著。Objective:To investigate the effects of Topiramate combined with Sodium Valproate on cognitive function and serum inflammatory factors and so on in patients with epilepsy.Method:One hundred and eight patients with epilepsy who were admitted to Wuxue Traditional Chinese Medicine Hospital from January 2020 to January 2021 were selected and divided into observation group(n=54)and control group(n=54)using the random number table method.The control group was treated with Sodium Valproate,and the observation group was treated with Topiramate on the basis of the control group,and the cognitive function scores,serum inflammatory factor indexes,neurotrophic indexes and event-related evoked potential P300 wave-related values were compared between the two groups.Result:After 6 months of treatment,the brain-derived neurotrophic factor(BDNF),nerve growth factor(NGF)and event-related evoked potential P300 wave amplitudes in the observation group were higher than those in the control group,and the tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),central neurospecific protein(S100β),and event-related response time and latency of evoked potential P300 wave were lower than those of the control group,and Montreal cognitive inventory(MoCA)score was higher than that of the control group,and the differences were all statistically significant(P<0.05).Conclusion:Topiramate combined with Sodium Valproate in the treatment of epilepsy can improve the cognitive function,reduce the inflammatory response and improve the neurotrophic status of patients with significant effects.
