单孔胸腔镜与三孔胸腔镜下肺段切除术治疗非小细胞肺癌的近期效果Meta分析

摘　　要：目的:本研究探讨单孔胸腔镜与三孔胸腔镜下肺段切除术治疗非小细胞肺癌的近期效果。方法:计算机检索PubMed、Cochrane Library、Embase、中国知网数据库(CNKI)、维普全文期刊数据库和万方数据库等,检索时限为建库至2021年12月30日。收集有关单孔胸腔镜与三孔胸腔镜下肺段切除术治疗非小细胞肺癌的对比研究,由2名评价员分别独立对纳入文献进行资料提取,用Revman 5.3软件进行Meta分析。结果:共纳入6项回顾性临床研究,共418例患者,Meta分析结果显示,单孔胸腔镜与三孔胸腔镜下肺段切除术治疗非小细胞肺癌相比,术中出血量少[MD=-23.50,95%CI(-24.70,-22.29),P<0.01],术后住院时间短[MD=-2.06,95%CI(-2.28,-1.83),P<0.01],手术时间长[MD=18.82,95%CI(15.30,22.35),P<0.001],差异均有统计学意义(P<0.05)。两者淋巴结清扫个数MD=[0.05,95%CI(-0.22,0.33),P=0.70]及胸腔引流时间[MD=0.11,95%CI(-0.08,0.29),P=0.26]比较,差异均无统计学意义(P>0.05)。结论:与传统三孔胸腔镜肺段切除术相比,单孔胸腔镜在肺段手术具有相似的手术治疗效果,更低的术中出血风险和更快的术后康复速度。Objective:To investigate the short-term efficacy between uniportal and multiportal video-assisted thoracoscopic surgery(VATS)segmentectomy in the treatment of non-small cell lung cancer(NSCLC).Method:By searching PubMed,the Cochrane Library,Embase,CNKI database,VIP full-text journal database and Wanfang database,all the comparative study of uniportal and multiportal VATS segmentectomy in the treatment of NSCLC were collected.The retrieval time limit was from the establishment of the database to December 30,2021.The data of the included literature were extracted independently by two evaluators,and Meta-analysis was carried out by Revman 5.3 software.Result:A total of 418 patients in 6 retrospective clinical studies were included.Meta analysis results showed that,compared with multiportal VATS segmentectomy,intraoperative bleeding volume of the uniportal VATS segmentectomy was less[MD=-23.50,95%CI(-24.70,-22.29),P<0.01],postoperative hospital stay was shorter[MD=-2.06,95%CI(-2.28,-1.83),P<0.01]and the operation time was longer MD=[18.82,95%CI(15.30,22.35),P<0.001],there were statistically significant(P<0.05).The number of lymph node dissection[MD=0.05,95%CI(-0.22,0.33),P=0.70]and thoracic drainage time[MD=0.11,95%CI(-0.08,0.29),P=0.26]had no significant differences(P>0.05).Conclusion:Compared with traditional multiportal VATS segmentectomy,uniportal VATS segmentectomy has similar surgical treatment effect,lower intraoperative bleeding risk and faster postoperative rehabilitation.
