脾胃培元散穴位贴敷防治乳腺癌化疗后消化道反应的临床研究

摘　　要：目的:观察乳腺癌化疗导致的恶心呕吐在应用脾胃培元散联合5-HT3受体拮抗剂及皮质类固醇药物防治后的疗效。方法:临床纳入我院肿瘤科2020年6月~2022年4月收治的57例乳腺癌化疗患者,随机分为贴敷组(28例)和对照组(29例)。两组均于化疗d1~d3予以昂丹司琼、地塞米松预处理,贴敷组在对照组基础上加用脾胃培元散贴敷神阙,并予TDP灯照射药袋40min,化疗第1-7d每日一次,7d一个疗程。分别比较贴敷组及对照组化疗后对恶心、呕吐的控制情况,同时比较两组患者化疗前后营养指数(PNI)及生活质量(KPS)的变化。结果:化疗后贴敷组患者恶心、呕吐(急性及延迟)的情况较对照组轻;化疗后贴敷组PNI、KPS评分高于化疗前,化疗后对照组PNI、KPS评分低于化疗前,化疗后贴敷组PNI、KPS评分明显高于对照组。结论:脾胃培元散联合昂丹司琼及地塞米松能够改善乳腺癌化疗者的恶心、呕吐,同时能够提高机体营养状态并能改善生活质量。Objective To determine the efficacy of Piweipeiyuan San combination with 5-HT3 inhibitor and steroids in the diagnosis and removal of nauseous vomiting due to chemotherapy for breast cancer.Methods 57 breast cancer chemotherapy recipients were invited to our cancer service from June 2020 to April 2022,and randomly grouped into the post chemotherapy opplication group(28 cases) and the control group(29 cases),both groups were given ondansetron and dexamethasone pretreatment on d1-3 of chemotherapy,the application group received Piweipeiyuan San application on Shen Que point and TDP lamp for 40 min on the basis of control group,once a day on d1-7 of chemotherapy,7d a session.The regulation of nauseous and vomiting after chemotherapy was verified between two groups,and the variations of PNI and KPS metrics around chemotherapy were also verified among the two groups.Results The nausea and vomiting (acute and delayed) of the patients in the application group after chemotherapy were lighter than those in the control group;PNI and KPS scores in the post chemotherapy application group were higher than those before chemotherapy,and PNI and KPS scores in the post chemotherapy application group were lower than those before chemotherapy.PNI and KPS scores in the post chemotherapy application group were significantly higher than those in the control group.Conclusion Piweipeiyuan San together with ondansetron and dexamethasone can alleviate nauseous and vomiting signs in breast cancer chemotherapy patients,as effectively as PNI and KPS.
