摘　　要：目的:探究下颌水平低位阻生齿拔除术治疗下颌阻生第三磨牙对患者牙周健康状况的影响。方法:采用随机数字表法将2021年2月~2022年3月160例下颌水平低位阻生第三磨牙患者分为两组,每组80例。对照组给予传统凿骨劈冠法治疗,观察组给予高速涡轮钻微创拔牙法治疗。比较两组手术成功率、术后恢复情况及治疗前后牙槽骨相关指标、牙周健康指标,并采用口腔健康影响程度量表-14(OHIP-14)评价患者远期生活质量。结果:两组手术成功率均为100%;观察组拔牙时间、牙槽窝愈合时间短于对照组,术后24 h VAS评分、肿胀程度低于对照组,开口受限度低于对照组;两组患者治疗后牙槽骨相关指标及牙周健康指标比较差异无统计学意义;观察组患者治疗后功能限制、生理性疼痛及OHIP-14总分均低于对照组;两组患者术后并发症发生率比较无明显差异。结论:相比传统拔牙术,采用高速涡轮钻微创拔牙法治疗下颌水平低位生第三磨牙,可降低患者术后疼痛及牙周肿胀程度,改善其远期生活质量,但两种治疗方法对患者牙周健康状况的影响不大。Objective To investigate the effects of mandibular horizontal low impacted tooth extraction on the periodontal health status in the treatment of patients with impacted mandibular third molars.Methods 160 patients with mandibular horizontal low impacted third molars from February 2021 to March 2022 were divided into two groups by random number table method,with 80 cases in each group.The control group was given traditional chiseled bone splitting crown method,and the observation group adopted minimally invasive tooth extraction with high-speed turbodrill.The surgical success rate,postoperative recovery,alveolar bone-related indicators and periodontal health indicators before and after treatment were compared between the two groups,and Oral Health Impact Profile-14(OHIP-14) was used to evaluate the long-term quality of life of patients.Results The surgical success rates of the two groups were both 100%.The tooth extraction time and alveolar fossa healing time in the observation group were shorter than those in the control group,and the VAS score and swelling degree at 24 h after surgery were lower than those in the control group,and the opening restriction was low compared to the control group.There were no statistical differences in alveolar bone-related indicators and periodontal health indicators between the two groups after treatment.The scores of functional limitation and physiological pain and total score of OHIP-14 after treatment were lower in the observation group than the control group.There was no significant difference in the incidence rate of postoperative complications between the two groups.Conclusion Compared with traditional tooth extraction,high-speed turbodrill minimally invasive tooth extraction in the treatment of horizontal low mandibular third molars can relieve the postoperative pain and periodontal swelling and improve the long-term quality of life,but the two treatment methods have little effect on the periodontal health status of patients.
