摘　　要：目的 应用N-NHPET/CT研究帕金森病(PD)患者大脑皮质区域脑血流(rCBF)与疾病严重程度的相关性,寻找评估疾病严重程度的新方法。方法 收集PD患者的一般临床资料和N-NHPET/CT的显像资料,并计算大脑皮层各区域的标准摄取比(SUVr)。采用国际运动障碍协会统一制定的帕金森病综合评定量表(MDS-UPDRS)及Hoehn-Yahr(H-Y)分期评估PD患者临床症状及病情严重程度。采用Pearson相关或Spearman秩相关回顾性分析患者病情严重程度与脑皮质区域血流的关系。结果 回顾性分析入组患者共23例,其中直回、额下回、眶内回、眶外侧回、眶后回的SUVr值与H-Y分期呈负相关性(r=-0.418、-0.437、-0.414、-0.620、-0.493,P<0.05);额下回、眶前回、眶外侧回、顶上回、扣带回前部及扣带回后部的SUVr与MDS-UPDRSⅢ运动评分均呈相关性(r=-0.439、-0.435、-0.460、-0.450、-0.422,-0.522,P<0.05)。运动迟缓与眶外侧回、顶上回、扣带回后部呈负相关性(r=-0.494、-0.476、-0.500,P<0.05);姿势稳定性/步态障碍评分与直回、眶外侧回、顶上回、舌回、扣带回前部呈负相关性(r=-0.468、-0.586、-0.417、-0.437、-0.454,P<0.05)。强直评分与直回、额下回、眶内回、眶外侧回、顶上回、扣带回前部、扣带回后部的SUVr呈负相关性(r=-0.503、-0.432、-0.428、-0.524、-0.446、-0.614、-0.554,P<0.05),与前颞叶外侧、颞中下回呈正相关性(r=0.505、0.426,P<0.05)。震颤评分与大脑皮质的各脑区的SUVr均呈负相关性。结论 大脑皮质区域血流的变化可以作为监测PD严重程度新的生物学指标。Objective To study the was used To study the correlation between regional cerebral blood flow(rCBF)and disease severity in cerebral cortex of patients with Parkinson′s disease(PD)by usingN-NHPET/CT,and to find a new method to evaluate the severity of disease.Methods The general clinical data andN-NHPET/CT imaging data of PD patients were collected and calculated the standardized uptake value ratio(SUVr)of various cortical regions.The clinical symptoms and severity of PD patients were assessed by the Movement Disorder Society-Unified Parkinson′s Disease Rating Scale(MDS-UPDRS)and Hoehn and Yahr(H-Y) stages.Pearson correlation or Spearman rank correlation were used to retrospectively analyze the relationship between the severity of the disease and the blood flow in the cortical region.Results A total of 23 patients were retrospectively analyzed.The SUVr values of straight gyrus,inferior frontal gyrus,internal orbital gyrus,lateral orbital gyrus and posterior orbital gyrus were negatively correlated with H-Y stage(r=-0.418,-0.437,-0.414,-0.620,-0.493,P<0.05).SUVr of inferior frontal gyrus,anterior orbital gyrus,lateral orbitalgyrus,superior parietal gyrus,anterior cingulate gyrus and posterior cingulate gyrus were negatively correlated with MDS-UPDRSⅢ motor score(r=-0.439,-0.435,-0.460,-0.450,-0.422,-0.522,P<0.05).The bradykinesia was negatively correlated with lateral orbital gyrus,superior parietal gyrus and posterior cingulate gyrus(r=-0.494,-0.476,-0.500,P<0.05).Postural stability/gait disorder scores were negatively correlated with straight gyrus,lateral orbital gyrus,superior parietal gyrus,lingual gyrus and anterior cingulate gyrus(r=-0.468,-0.586,-0.417,-0.437,-0.454,P<0.05).The rigidity score was negatively correlated with SUVr of straight gyrus,inferior frontal gyrus,intraorbital gyrus,lateral orbital gyrus,superior parietal gyrus,anterior cingulate gyrus,and posterior cingulate gyrus(r=-0.503,-0.432,-0.428,-0.524,-0.446,-0.614,-0.554,P<0.05).It was positively correlated with lateral anterior tempora
