清肺化痰通腑方治疗脑出血合并肺部感染痰热蕴肺证疗效观察

摘　　要：目的观察清肺化痰通腑方治疗脑出血合并肺部感染痰热蕴肺证患者的效果。方法将陕西中医药大学附属医院2020年12月—2021年12月收治的72例脑出血合并肺部感染患者随机分为对照组与治疗组各36例。其中对照组给予院内常规西药治疗,治疗组在对照组治疗基础上予以清肺化痰通腑方治疗,2组患者均连续治疗2周。观察2组患者治疗前及治疗后第7,14天肺部感染评分(CPIS)、白细胞计数(WBC)、C反应蛋白(CRP)、降钙素原(PCT)水平,比较2组治疗后中医症状疗效及临床症状体征消失或恢复正常时间。结果2组患者治疗第7,14天后的CPIS评分和血WBC、CRP、PCT水平均较治疗前明显降低(P均<0.05),治疗组各指标较对照组降低更明显(P均<0.05);治疗后治疗组总有效率为91.7%(33/36),显著高于对照组的80.6%(29/36),2组比较差异有统计学意义(P<0.05);治疗组咳嗽消失时间、肺部湿啰音消失时间、体温恢复正常时间及WBC恢复正常时间均明显短于对照组(P均<0.05)。结论西药联合清肺化痰通腑方治疗脑出血合并肺部感染可明显提高治疗有效率,改善患者肺部感染症状,有效降低血清炎症因子水平。Objective It is to observe the curative effect of Qingfei Huatan Tongfu decoction in the treatment of cerebral hemorrhage complicating with pulmonary infection with syndrome of phlegm heat accumulating in the lung.Methods Seventy-two patients with cerebral hemorrhage and pulmonary infection admitted to the Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine from December 2020 to December 2021 were randomly divided into control group and treatment group,with 36 patients in each group.The control group was treated with routine western medicine in the hospital,and the treatment group was treated with Qingfei Huatan Tongfu Decoction on the basis of therapy in the control group.The patients in both groups were treated continuously for 2 weeks.The clinical pulmonary infection scores(CPIS),levels of white blood cell count(WBC),C-reactive protein(CRP),and procalcitonin(PCT)of the patients in the two groups were observed before treatment and on the 7th and 14th days after treatment,and the curative effect of TCM symptoms,and the clinical symptoms and signs disappear time or return to normal time were compared between the two groups after treatment.Results The scores of CPIS and the levels of serum WBC,CRP and PCT of the patients in the two groups after 7 and 14 days of treatment were significantly lower than those before treatment(all P<0.05),and the indexes in the treatment group were significantly lower than those in the control group(all P<0.05).After treatment,the total effective rate of the treatment group was 91.7%(33/36),which was significantly higher than 80.6%(29/36)of the control group(P<0.05).The disappearance times of cough and lung moist rale,return to normal time of body temperature and WBC in the treatment group were significantly shorter than those in the control group(all P<0.05).Conclusion Western medicine combined with Qingfei Huatan Tongfu decoction can significantly improve the treatment efficiency,improve the symptoms of pulmonary infection,and effectively reduce the level of se
