超静脉溶栓时间窗前循环急性脑梗死应用恩必普注射液治疗的疗效观察

摘　　要：目的探讨超静脉溶栓时间窗(超窗)前循环急性脑梗死应用恩必普注射液治疗的疗效。方法216例超窗前循环急性脑梗死住院患者,随机分为对照组和观察组,每组108例。两组患者根据病情给予抗血小板聚集、稳斑及保护神经等常规治疗,观察组给予恩必普注射液静脉滴注治疗。比较两组患者治疗前后生化指标[血清超敏C反应蛋白(hs-CRP)、神经元特异性烯醇化酶(NSE)]水平,美国国立卫生研究院卒中量表(NIHSS)评分,Barthel指数(BI)评分,改良Rankin量表(mRS)评分。结果治疗后,观察组患者血清hs-CRP(3.92±1.56)mg/L、NSE(7.01±2.05)ng/ml均低于对照组的(5.15±1.07)mg/L、(8.98±2.18)ng/ml,差异有统计学意义(P<0.05)。治疗14 d后,两组患者NIHSS、mRS评分低于治疗前,BI评分高于治疗前,且观察组患者NIHSS评分(8.05±2.15)分、mRS评分(2.75±0.46)分低于对照组的(10.52±1.37)、(3.21±0.86)分,BI评分(89.87±12.64)分高于对照组的(73.18±14.70)分,差异均具有统计学意义(P<0.05)。结论恩必普注射液对超窗前循环急性脑梗死患者治疗效果较佳,可促进受损伤神经功能的恢复,改善日常生活能力,改善预后。Objective To discuss the efficacy of Enbipu injection on acute cerebral infarction in anterior circulation beyond the time window of intravenous thrombolysis.Methods A total of 216 hospitalized patients with acute anterior circulation cerebral infarction beyond the time window of intravenous thrombolysis were randomly divided into control group and observation group,with 108 cases in each group.Patients in the two groups received conventional treatments such as anti-platelet aggregation,plaque stabilization and nerve protection according to their conditions,and the observation group received intravenous drip of Enbipu injection.Both groups were compared in terms of biochemical indexes[serum high-sensitivity C-reactive protein(hs-CRP),neuron-specific enolase(NSE)],National Institutes of Health Stroke Scale(NIHSS)score,Barthel index(BI)score,modified Rankin scale(mRS)score before and after treatment.Results After treatment,the serum hs-CRP(3.92±1.56)mg/L and NSE(7.01±2.05)ng/ml of observation group were lower than(5.15±1.07)mg/L and(8.98±2.18)ng/ml ml of control group,and the differences were all statistically significant(P<0.05).After 14 d of treatment,the NIHSS score and mRS score of the two groups were lower than those of this group before treatment,and the BI score was higher than those of this group before treatment;the NIHSS score(8.05±2.15)points and mRS score(2.75±0.46)points of the observation group were lower than(10.52±1.37)and(3.21±0.86)points of the control group;the BI score(89.87±12.64)points of the observation group was higher than(73.18±14.70)points of the control group;the differences were all statistically significant(P<0.05).Conclusion Enbipu injection is effective for patients with acute anterior circulation cerebral infarction beyond the time window of intravenous thrombolysis,and can promote the recovery of injured nerve function,improve the ability of daily living and improve the prognosis.
