摘  要:目的 探讨血液中神经元特异性烯醇化酶(NSE)、D-二聚体(D-D)、同型半胱氨酸(Hcy)在脑出血病情评估中的价值。方法 选取2017年1月—2019年12月亳州市人民医院收治的高血压脑出血患者80例为脑出血组。根据入院当日格拉斯哥昏迷指数(GCS)将脑出血组患者分为轻度组(35例)、中度组(23例)、重度组(22例)。选取同期来本院体检的30例健康成年人为对照组。对照组入院当日,脑出血组发病后1、3、7 d时,清晨采集静脉血,检测血液中NSE、D-D、Hcy水平。比较对照组与脑出血组患者血液NSE、D-D、Hcy水平。比较轻度组、中度组、重度组脑出血患者发病后1、3、7 d时的血液NSE、D-D、Hcy水平。对血液NSE、D-D、Hcy水平与出血量进行相关性分析。结果 脑出血组发病后各时间点的血液NSE、D-D、Hcy水平均显著高于对照组(P<0.05)。中重度组的血液NSE、D-D、Hcy水平均显著高于轻度组(P<0.05);重度组的血液NSE、D-D、Hcy水平均显著高于中度组(P<0.05)。脑出血组出血量与血液NSE、D-D、Hcy水平呈显著正相关关系(P<0.05)。结论 高血压脑出血患者血液NSE、D-D、Hcy水平与脑损伤程度和血肿量密切相关。ObjectiveTo explore the values of serum neuron-specific enolase(NSE),D-dimer(D-D),and homocysteine(Hcy)in the assessment of cerebral hemorrhage.MethodsEighty patients with hypertensive cerebral hemorrhage were divided into 3 groups:mild group(n=35),moderate group(n=23)and severe group(n=22)according to the Glasgow Coma Scale(GCS)score on admission.Thirty healthy adults for physical examination during the same period were selected as control group.Fasting peripheral venous blood samples were collected from the control group on admission,and from the cerebral hemorrhage group 1,3 and 7 days after onset to test the serum levels of NSE,D-D,and Hcy by chemoluminescence method,automatic coagulation analyzer,and automatic biochemical analyzer respectively.Pearson linear correlation analysis was used to study the correlations of NSE,D-D,and Hcy with the bleeding volume.ResultsThe serum NSE,D-D,and Hcy levels at different time points of the cerebral hemorrhage group were all significantly higher than those of the control group(all P<0.05).The serum NSE level 3 d after onset of the cerebral hemorrhage group was the highest,and the serum NSE level 7 d after onset was significantly lower than that 3 d after onset(P<0.05).The serum NSE,D-D,and Hcy levels at different time points of the moderate and severe groups were all significantly higher than those of the mild group(all P<0.05),and the serum NSE levels at different time points of the severe groups were all significantly higher than those of the moderate group(all P<0.05).The serum D-D,and Hcy levels 3 d after onset of the cerebral hemorrhage group was significantly higher than those 1 d after onset.(both P<0.05),and the serum D-D and Hcy levels 7 d after onset were significantly higher than those 3 d after onset(both P<0.05).Pearson correlation analysis showed that the average bleeding volumes of the cerebral hemorrhage group 1,3,and 7 d after onset were all significantly positively correlated with the serum levels of NSE,D-D,and Hcy at those time points(all P<0.05).Concl
