摘  要:目的 探讨前列腺健康指数(PHI)联合F-FDG PET/CT检查的最大标准摄取物(SUVmax)对前列腺癌早期诊断及恶性程度的评估价值。方法 前瞻性选取2019年12月至2021年12月在我院就诊的疑似前列腺癌患者116例,检测患者血清总前列腺特异抗原(tPSA)、游离前列腺特异性抗原(f PSA)、前列腺特异抗原前体2型(p2PSA)、PHI及F-FDG PET/CT检查的SUVmax。结果 最终经会阴前列腺12针细针穿刺活检病理确诊,确诊为前列腺癌患者70例,良性前列腺增生患者46例。前列腺癌患者tPSA、fPSA、p2PSA及PHI分别为(35.58±16.69)ng/mL、(5.01±1.15)ng/mL、(60.30±21.1)ng/mL和(214.12±98.28),明显高于良性前列腺增生患者(P<0.05)。tPSA、fPSA、p2PSA及PHI。诊断为前列腺癌的ROC曲线下面积分别为0.800、0.763、0.846和0.906,P<0.05。前列腺癌患者通过F-FDG PET/CT检查显示SUVmax为(8.92±2.54),明显高于良性前列腺增生患者(P<0.05)。PHI联合F-FDG PET/CT检查的SUVmax诊断前列腺癌的ROC曲线下面积为0.953,P<0.05。前列腺癌TNM分期Ⅲ~Ⅳ期患者tPSA、fPSA、p2PSA、PHI及F-FDG PET/CT检查的SUVmax分别为(38.85±10.05)ng/mL、(5.41±0.98)ng/mL、(66.07±12.28)ng/mL、(239.39±67.79)和(10.19±1.65),明显高于Ⅰ~Ⅱ期患者(P<0.05)。结论 PHI值联合F-FDG PET/CT检查的SUVmax值诊断前列腺癌有较好的价值,同时在评估晚期前列腺癌恶性程度方面有一定作用。Objective To investigate the significance of prostate health index(PHI) combined with maximum standard uptake(SUVmax) ofF-FDG PET/CT in the early diagnosis of prostate cancer and the evaluation of malignant degree.Methods A total of 116 patients with suspected prostate cancer who were admitted to our hospital from December 2019 to December 2021 were prospectively recruited. Serum total prostate specific antigen(tPSA), free prostate specific antigen(fPSA), prostate specific antigen precursor type 2(p2PSA) of the patients were measured, their PHI and SUVmax ofF-FDG PET/CT were analyzed. Results The final diagnosis was pathologically confirmed through 12-needle fine needle biopsy of the prostate. Seventy patients were diagnosed as prostate cancer, and 46 patients as benign prostatic hyperplasia. The levels of t PSA, fPSA, p2PSA and PHI in patients with prostate cancer were(35.58±16.69)ng/mL,(5.01±1.15)ng/mL,(60.30±21.1)ng/mL and(214.12 ± 98.28), respectively, which were all significantly higher than those in patients with benign prostatic hyperplasia(P<0.05). The areas under the ROC curve for prostate cancer were 0.800, 0.763, 0.846, and 0.906, respectively(P<0.05). TheF-FDG PET/CT scan of patients with prostate cancer showed that the SUVmax was(8.92±2.54), which was significantly higher than that of patients with benign prostatic hyperplasia(P<0.05). The area under the ROC curve of PHI combined with SUVmax in the diagnosis of prostate cancer was 0.953, P<0.05. The SUVmax values of tPSA, f PSA, p2PSA,PHI andF-FDG PET/CT in patients with TNM stage Ⅲ–Ⅳ prostate cancer are(38.85 ± 10.05)ng/mL,(5.41 ± 0.98)ng/mL,(66.07 ± 12.28)ng/mL,(239.39 ± 67.79) and(10.19 ± 1.65), respectively, which are significantly higher than those in patients with stage I–II prostate cancer(P<0.05). Conclusion PHI combined with SUVmax valueshows a good diagnostic value for prostate cancer, and also plays a certain role in evaluating the degree of malignancy.
