摘　　要：目的:评估胶囊内镜在消化系统疾病中的诊断价值。方法:回顾性分析2008年3月至2019年1月中山大学孙逸仙纪念医院胶囊内镜检查的688例患者的临床资料。结果:688例患者中,检查成功率为99.71%;对上消化道、小肠及结肠疾病检出率分别为37.90%、58.89%及6.12%;对小肠疾病的检出率,男性、青年克罗恩病、消瘦、消化道出血明显高于其他患者(P<0.05),且明显优于CT、MR和(或)全消化道钡餐检查(P<0.05)。胶囊内镜的诊断与临床诊断符合率为47.23%,对消化道出血、不明原因消化道出血有明显优势(P<0.05)。结论:胶囊内镜是一种有效的消化系统疾病检查手段,尤其对小肠疾病及消瘦、消化道出血的患者有较高的诊断价值。Objective:To investigate the diagnostic value of capsule endoscopy in digestive diseases. Methods:The clinical data of 688 cases undergoing capsule endoscopy from Mar 2008 to Jan 2019 in Sun Yat-sen Memorial Hospital of Sun Yat-sen University were retrospectively analyzed. Results:Among the 688 cases,99.71% successfully completed the examination. The detection rates of diseases in upper gastrointestinal tract,small intestine and colon were37.90%,58.89% and 6.12% respectively. The detection rates were higher in male,young patients with Crohn’s disease,those complained with weight loss or gastrointestinal bleeding than other patients(P<0.05). Capsule endoscopy was superior to CT,MR and(or)gastrointestinal barium meal examination in detecting positive lesions(P<0.05). The coincidence rate between capsule endoscopy diagnosis and clinical diagnosis was 47.23%,among which patients with gastrointestinal bleeding and unexplained gastrointestinal bleeding showed the highest diagnostic coincidence rate(P<0.05). Conclusion:Capsule endoscopy is an effective method especially in the diagnosis of small intestinal diseases. It represents high diagnostic value for patients with weight loss and gastrointestinal bleeding.
