摘 要:目的探讨脑卒中高危人群血清髓系细胞触发受体-1(serum triggering receptor expressed on myeloid cells-1,s TREM-1)与颈动脉狭窄的相关性。方法回顾性分析余姚市人民医院脑卒中高危患者87例;并选择同期健康对照18例。按照s TRAM-1水平将脑卒中高危患者组分为第一分位数、第二分位数和第三分位数3组,并检查颈动脉血管狭窄程度。结果脑卒中高危组中收缩压、血糖和s TREM-1水平高于对照组(P<0.05),而HDL水平低于对照组(P<0.05)。与无狭组相比,轻度、中重度狭窄组s TREM-1水平呈递增趋势(P<0.05),并且轻度、中重度狭窄组的第一分位数组的s TREM-1所占百分比呈下降趋势(P<0.05)。相关性分析显示,s TREM-1水平与动脉狭窄程度成正相关(r=0.292,P=0.000)。多因素logistic回归分析表明,s TREM-1是颈动脉狭窄病变的独立危险因素(P<0.05)。ROC结果显示,s TREM-1联合收缩压预测动脉狭窄的曲线下面积(AUC)为0.831,灵敏度为0.724,特异度为0.778。结论s TREM-1是脑卒中高危人群颈动脉狭窄的病变的独立危险因素,s TREM-1联合收缩压检测有助于预测颈动脉狭窄病变。Objective To investigate the relationship between serum triggering receptor expressedon myeloid cells-1(s TRAM-1)and carotid stenosis in high risk stroke population.Methods A retrospective study was conducted on the clinical data of 87cases with high risk of stroke,and 18 healthy cases were selected into the control group.Patients at high risk of stroke were divided into the first,second and third quantile groups according to the level of s TRAM-1,and the degree of carotid artery stenosis was examined.Results The levels of systolic blood pressure,blood glucose and s TREM-1 in the high-risk stroke group were significantly higher than those in the control group(P<0.05),while the level of HDL was significantly lower than that in the control group(P<0.05).Compared with the non-stenosis group,the level of s TREM-1 in the mild and moderate to severe stenosis groups showed a gradual increasing trend(P<0.05),and the percentage of s TREM-1 in the first quantile array in the mild and moderate to severe stenosis groups showed a gradual decreasing trend(P<0.05).Correlation analysis showed that s TREM-1 level was significantly positively correlated with the degree of arterial stenosis(r=0.292,P=0.000).Multivariate logistic regression analysis showed that s TREM-1 was an independent risk factor for carotid artery stenosis(P<0.05).ROC results showed that the area under curve(AUC)of s TREM-1 combined with systolic blood pressure in predicting arterial stenosis was 0.831,the sensitivity was 0.724,and the specificity was 0.778.Conclusion s TREM-1 level is an independent risk factor for carotid artery stenosis in high-risk stroke population.s TREM-1 combined with systolic blood pressure measurement is helpful to predict carotid artery stenosis.
