摘　　要：患者女,35岁,因全身红色斑块、结节5个月,皮损加重半个月入院。患者5个月前无明显诱因出现头皮、胸腹部红斑、斑块和结节,无明显自觉症状,皮损逐渐增多。半个月前皮损迅速增多并融合,累及全身,伴发热、乏力、肌肉酸痛、活动后气促及吞咽困难。皮肤科检查:面部、躯干及四肢多发融合性斑块、结节,浸润明显,质硬,表面光亮。右侧腋下、双侧腹股沟可触及淋巴结增大。口腔上颚及双侧颊黏膜可见黄白色斑块。皮损组织病理检查及免疫组化结果提示:考虑诊断母细胞性浆细胞样树突状细胞肿瘤。正电子发射体层成像(PET-CT)结果提示:考虑淋巴瘤伴多系统受累可能性大。A 35-year-old female patient was admitted to hospital for red plaques and nodules all over the body for 5 months with acute progression for half a month.Five months before admission,erythema,plaques,nodules began to appear on her scalp,chest,and abdomen with no obvious cause;no obvious discomfort was felt and the skin lesions gradually increased.Two weeks before admission,the skin rash increased rapidly and merged,involving the whole body and accompanied by fever,fatigue,muscle aches,shortness of breath after physical activity,and dysphagia.Physical examination showed multiple fusion of plaques and nodules on the face,trunk,and extremities,with hard,and bright surface and obvious infiltration.Palpable swollen lymph nodes were found in the right armpit and bilateral groin.Yellow-white plaques could be seen on the palate and cheek membranes.Histopathological and immunohistochemical results suggested blast plasmacytoid dendritic cell tumors.The results of PET-CT indicated lymphoma with multiple system involvement.
