严光俊教授治疗溃疡性结肠炎的用药规律研究

摘　　要：目的:基于数据挖掘探究严光俊教授治疗溃疡性结肠炎的处方用药经验。方法:收集2020年1月-2021年12月严光俊教授治疗溃疡性结肠炎的有效医案处方,经过数据筛选、清洗、标准化处理,建立标准化医案数据库,运用数据挖掘方法分析严师治疗溃疡性结肠炎的用药规律。结果:本研究共纳入处方127首,涉及药物79味,高频药物为茯苓、法半夏、黄连、甘草、麸炒白术等,四气以温、平、寒三性为主,五味以苦、甘、辛三味为主,归经以脾、胃、肺经为主,功效以补气药、清热燥湿药、利水渗湿药三类为主,出现频数最多的药对为法半夏-茯苓。结论:严师治疗溃疡性结肠炎多从中焦脾胃着手,重视气机升降;培补脾胃以治其本,清化通降以治其标;清温并用以和阴阳,辛开苦降以调升降。Objective:To investigate Professor YAN Guangjun’s prescribing experience in treating ulcerative colitis based on data mining.Methods:It was collected that the effective medical records and prescriptions of Professor YAN Guangjun from January 2020 to December 2021 for the treatment of ulcerative colitis.After data screening,cleaning and standardized processing,a standardized medical record database was established,and a data mining algorithm was used to analyze the medication rules of Professor YAN in the treatment of ulcerative colitis.Results:Totally 127 prescriptions were included in this study,involving 79 kinds of TCM medicines.Highfrequency medicines included Fuling(Poria),Fa Banxia(Pinellia),Huanglian(Coptidis),Gancao(Licorice),Fuchao Baizhu(Bran fried atractylodes),etc..The medicinal properties were mainly warm,moderate and cold.The medicinal flavors were mainly bitter,sweet,and spicy.The meridian tropism was mainly spleen,stomach and lung meridians.According to the classification of efficacy,there were mainly three categories:the Qi-invigorating(补气)medicine,the heat-clearing and dampness-removing medicine,and the dampness-excreting medicine.The most frequent combination was Fa Banxia-Fuling.Conclusion:In the treatment of ulcerative colitis,Professor YAN mostly starts with the spleen and stomach of the middle energizer,and pays attention to the rise and fall of Qi.The method of tonifying spleen and stomach is used to eliminate the root cause,supplemented by clearing gastrointestinal tract and boost digestion to alleviate manifested symptoms.The method of clearing and warming simultaneously is used to reconcile Yin(阴)and Yang(阳).Meanwhile,the therapy of acrid opening and bitter downbearing is used to adjust the rise and fall of Qi.
