TEAD4、SHP-2及Ang-2表达与口腔鳞癌病理参数和预后的关系

摘　　要：目的 分析组蛋白赖氨酸去甲基化酶4A(TEAD4)、蛋白酪氨酸磷酸酶-2(SHP-2)及促血管生成素-2(Ang-2)表达与口腔鳞癌(OSCC)病理参数、预后的关系。方法 收集2019年12月至2020年12月于河北北方学院附属第一医院行外科手术治疗或活检的OSCC患者121例,以患者术中病理送检组织作为OSCC组,癌旁组织(距离癌组织>5 cm)作为对照组。收集患者临床资料,分析不同组织中TEAD4、SHP-2及Ang-2表达情况;对患者进行为期12个月随访,根据预后情况分预后良好与预后不良组,了解三因子与患者病理特征及预后的关系。结果 OSCC癌组织中TEAD4、SHP-2及Ang-2阳性表达均明显高于对照组,差异均有统计学意义(P<0.05)。TEAD4在有淋巴结转移、临床分期为Ⅲ~Ⅳ期患者中阳性表达占比高于无淋巴结转移、临床分期为Ⅰ~Ⅱ期患者,SHP-2在有淋巴结转移、临床分期为Ⅲ~Ⅳ期、组织分化为低分化患者中阳性表达占比高于无淋巴结转移、临床分期为Ⅰ~Ⅱ期、组织分化为中高分化患者,淋巴结转移者Ang-2阳性表达明显高于淋巴结未转移者,差异均有统计学意义(P<0.05)。121例患者预后良好87例,预后不良34例,预后不良组临床分期为Ⅲ~Ⅳ期、组织分化为低分化、有淋巴结转移患者组织中TEAD4、SHP-2、Ang-2阳性表达明显高于预后良好组,差异均有统计学意义(P<0.05)。经多元Logistic回归分析可知,临床分期、组织分化、淋巴结转移、TEAD4、SHP-2、Ang-2是影响OSCC患者预后不良的独立危险因素(P<0.05)。结论 TEAD4、SHP-2及Ang-2与患者病理特征及预后均存在相关性,三者或许可成为OSCC诊断及预后的相关肿瘤标记物,对临床诊疗方案制定也有一定参考价值。Objective To analyze the relationship between Histone Lysine demethylase 4A(TEAD4),Protein Tyrosine Phosphatase-2(SHP-2)and Angiopoietin-2(Ang-2)expression,pathological parameters and prognosis of oral Squamous cell carcinoma(OSCC). Methods 121 patients with OSCC who underwent surgical treatment or biopsy in our hospital from December 2019 to December 2020 were enrolled,the pathological examination tissues of patients were selected as OSCC group,and the adjacent non-cancerous tissues were selected as control group. The adjacent tissue(more than 5 cm away from the cancer tissue)was used as the control group,the expressions of TEAD4,SHP-2 and ANG-2 in different tissues were analyzed.The patients were followed up for 12 months. According to the prognosis,they were divided into the good prognosis group and the poor prognosis group. Results The positive expressions of TEAD4,SHP-2 and ANG-2 in OSCC carcinoma tissues were significantly higher than those in the control group,and the differences were statistically significant(P<0.05). The proportion of positive expression of TEAD4 in patients with lymph node metastasis and clinical stageⅢ-Ⅳ was higher than that in patients without lymph node metastasis and clinical stageⅠ-Ⅱ. The proportion of positive expression of SHP-2 in patients with lymph node metastasis,clinical stageⅢ-Ⅳ,and poorly differentiated tissue is higher than that in patients without lymph node metastasis. In patients with clinical stageⅠ-Ⅱ and histologically differentiated into moderately and highly differentiated patients,the positive expression of Ang-2 in patients with lymph node metastasis was significantly higher than that in patients without lymph node metastasis,and the difference was statistically significant(P<0.05). Of the 121 patients,87 had good prognosis and 34 had poor prognosis.
