摘 要:目的探讨全牙弓种植即刻修复对患者的长期作用效果及并发症的危险因素。方法选取2020年1月―2021年2月本院收治的80例行全牙弓种植即刻修复治疗患者,对其临床资料及治疗情况进行回顾性分析,应用多因素Cox回归方法分析并发症危险因素。结果80例患者中,纳入修复体92个,其中下颌53个,上颌39个;即刻修复体戴用时间32-274周,平均(202.41±12.65)周;戴用时间>3年修复体13个,>5年者6个;修复体戴用6个月内,共13例患者13个修复体发生折裂,9例患者9个修复体发生松动;戴用全程中,共17例患者18个修复体发生折裂,16例患者17个修复体发生松动;戴用3、6、9个月的累积成功率分别为87.50%、87.14%、86.89%;折裂组性别、颌位、对颌、纤维加强与未折裂组比较,差异均有统计学意义(P<0.05);两组年龄、种植颗数相比,差异无统计学意义(P>0.05)。经多因素Cox回归模型分析显示,颌位(上)、固定对颌牙列、纤维加强与修复体并发症发生显著相关,差异均有统计学意义(P<0.05)。结论长期戴用全牙弓种植即刻修复体具有较高并发症发生率,于第1年内发生率最高,建议患者于术后6个月内更换为终末修复体。Objective To investigate the long-term effect of immediate restoration with total dental arch implantation on patients and the risk factors of complications.Methods From January 2020 to February 2021,80 patients treated with immediate restoration of total dental arch implantation in our hospital were selected as study subjects,their clinical data and treatment were studied retrospectively,and the risk factors of complications were analyzed by multivariate Cox regression method.Results In this group of 80 patients,92 prostheses were included in the study,including 53 of the lower jaw and 39 of the upper jaw.The wearing time of immediate prostheses ranged from 32 to 274 weeks,with an average of(202.41±12.65)weeks.13 prostheses were worn for more than 3 years and 6 prostheses were worn for more than 5 years.Within 6 months,13 prostheses in 13 patients were fractured and 9 prostheses in 9 patients were loosened.During the whole wearing process,18 prostheses in 17 patients were fractured and 17 prostheses in 16 patients were loosened.The success rates of wearing for 3,6 and 9 months were 87.50%,87.14%and 86.89%respectively.There were significant differences in sex,jaw position,opposite jaw and fiber reinforcement between the fracture group and the non fracture group(P<0.05),but there was no significant difference in age and number of implants between the two groups(P>0.05).According to the analysis of univariate Cox regression model,gender,jaw position,opposite jaw and fiber reinforcement may be the risk factors of prosthesis complications.Multivariate Cox regression model analysis showed that jaw position(upper),fixed opposite jaw dentition and fiber reinforcement were significantly correlated with prosthesis complications,the differences were statistically significant(P<0.05).Conclusions The long-term wearing of immediate prosthesis with total dental arch implantation results a high incidence of complications,and the incidence is the highest in the first year.It is suggested that the patients should be replaced wit
