亚急性期缺血性脑卒中PVS在近期临床预后评估中的应用研究

摘　　要：目的 探讨亚急性期缺血性脑卒中患者SWI中突出血管征(PVS)在近期临床预后评估中的应用价值。方法选取我院收治的60例急性缺血性脑卒中患者作为观察对象,于发病第3日及第7日完成MRI检查。依据发病第3日SWI序列中有无PVS显示,分为PVS阳性组(25例)与PVS阴性组(35例),比较两组患者发病第3日及第7日DWI序列提示梗死核心区范围,并采用美国国立卫生研究院卒中量表(NIHSS)于入院当日及第7日行神经功能缺失程度评分,并分析两组评分差异。结果 PVS阳性组患者发病第3日及第7日时梗死核心范围分别为(489.47±85.37) cm^(2)、(579.84±94.56) cm^(2),均高于PVS阴性组。PVS阳性组与阴性组入院当日NIHSS评分差异无统计学意义(P> 0.05),但PVS阳性组患者7日后NIHSS评分为(7.98±2.41),明显高于PVS阴性组,差异有统计学意义(P <0.05)。结论 亚急性期缺血性脑卒中患者于SWI序列显示的PVS阳性表现与患者近期不良预后相关。Objective To investigate the application value of magnetic susceptibility weighted imaging(SWI)with prominent vascular sign(PVS)in the evaluation of short-term clinical prognosis in patients with subacute ischemic stroke.Methods 60 patients with acute ischemic stroke in our hospital were selected as the research objects,and magnetic resonance imaging(MRI)examination was completed on the 3rd and 7th day of onset.The patients were divided into PVS positive group(25 cases)and PVS negative group(35 cases)according to the presence or absence of PVs in SWI sequence on the third day of onset.The diffusion weighted imaging(DWI)sequence on the 3rd and 7th day of onset in the two groups was compared.The degree of neurological deficit was evaluated by NIHSS on the day of admission and the 7th day.Results The infarct core area of PVS positive group was(489.47±85.37)cm^(2) and(579.84±94.56)cm^(2) on the 3rd and 7th day respectively,which were higher than those in PVS negative group.There was no significant difference in NIHSS score between PVS positive group and negative group on admission day(P>0.05),but NIHSS score of PVS positive group was(7.98±2.41)after 7 days,which was significantly higher than that of PVS negative group(P<0.05).Conclusion PVS positive expression in SWI sequence of patients with subacute ischemic stroke is related to the short-term adverse prognosis.
