摘 要:目的 探讨超小超顺磁性氧化铁(USPIO)动态增强MRI评价小鼠原位肝癌血流动力学的价值。方法 选用雄性Balb/c裸小鼠14只,随机分为高侵袭性组(n=7)和低侵袭性组(n=7),分别采用高侵袭性MHCC97H和低侵袭性Huh7人的肝癌细胞株构建肝癌原位移植瘤模型。使用USPIO进行3.0T MRI动态增强扫描,分别测量肿瘤平扫、增强后30 s、30 min、60 min及90 min的T_(1)和T_(2)值。比较两组肿瘤在不同时间点T_(1)和T_(2)值的差异,明确各组的达峰时间,比较两组的峰值差异。结果 高侵袭性组和低侵袭性组肿瘤的T_(1)达峰时间均为30 min,两组T_(1)信号峰值差异无统计学意义。高侵袭性组T达峰时间为60 min,低侵袭性组T_(2)达峰时间为30s。高侵袭性组的T_(2)强化峰值低于低侵袭性组(78.19±5.04 vs 85.59±13.78),两者差异具有统计学意义(P=0.02)。结论 双对比USPIO动态增强MRI可用于评价肝癌血流动力学,高侵袭性肝癌的T_(2)达峰时间延迟,负性强化程度增加。Objective To investigate the value of double-contrast ultra-small superparamagnetic iron oxide with dynamic enhancement MRI in evaluating the hemodynamics of liver cancer in mice.Methods Fourteen male Balb/c nude mice were randomly divided into a highlyaggressive group(n=7)and a lowaggressive group(n=7).High-invasive MHCC97H and low-invasive Huh7 liver cancer cell lines were applied to low and high-grade malignant group respectively for orthotopic transplantation tumor modelsconstruction.Contrast agent USPIO was used to perform dynamic contrast-enhanced 3.0T magnetic resonance imaging in two groups of animals.The signal values of T_(1) mapping and T_(2) mapping sequences were measured in mask phase,and at 30 s,30 min,60 min and 90 min after enhancement for different tumor groups respectively,for difference comparing between the low-aggressive and high-aggressive tumor group.In specific,we compared the difference in T_(1) and T_(2) values at different time points,determined the peak time of each group and compared the peak difference between the two groups.Results T_(1) values of the high-aggressive and low-aggressive tumors were both reduced to the lowest at 30 minutes after the enhancement.There was no significant difference in the T_(1) signal peak of two groups at 30min after the enhancement.In the T_(2) mapping sequence,the peak time of T_(2) in the highly aggressive group was 60 min,while the low aggressive group was 30s.The peak of enhancement in the highly aggressive group was lower than that in the low-aggressive group(78.19±5.04 vs 85.59±13.78).The difference between the two groups was statistically significant(P=0.02).Conclusion Double-contrast USPIO based DCE-MRI can be used to evaluate hemodynamics of liver cancer,with delayed T_(2) peak time and increased negative enhancement in highly aggressive liver cancer.
