摘 要:目的:探讨复发性口腔溃疡患儿治疗前后血清上皮细胞膜结合黏液素1(MUC1)、CC修饰趋化因子11(CCL11)水平变化及对复发的预测价值。方法:选取复发性口腔溃疡患儿136例为观察组,同期健康体检且口腔无溃疡者142例为对照组。比较复发性口腔溃疡患儿治疗前后各指标水平,比较未再次复发组和再次复发组各指标水平。采用酶联免疫吸附法(ELISA)测定血清MUC1、CCL11、肿瘤坏死因子(TNF-α)、白细胞介素2(IL-2)水平。Pearson法分析患儿血清MUC1、CCL11与TNF-α、IL-2的相关性。采用受试者工作特征(ROC)曲线评价血清MUC1、CCL11对复发性口腔溃疡再复发的预测价值。结果:与对照组相比,观察组血清MUC1、CCL11、TNF-α、IL-2水平显著升高(均P<0.05)。与治疗前相比,治疗后血清MUC1、CCL11、TNF-α、IL-2水平显著降低(均P<0.05)。Pearson法分析显示,血清MUC1、CCL11均与TNF-α、IL-2呈正相关(均P<0.05)。治疗后,与未再次复发组相比,再次复发组血清MUC1、CCL11、TNF-α、IL-2水平显著升高(均P<0.05)。ROC曲线分析表明,MUC1、CCL11预测复发性口腔溃疡再次复发的曲线下面积(AUC)为0.886、0.813;MUC1联合CCL11预测复发性口腔溃疡再次复发的AUC为0.908,敏感度为84.40%,特异性为86.50%。结论:治疗后复发性口腔溃疡患儿血清MUC1、CCL11水平显著降低,对预测复发性口腔溃疡患儿再次复发具有一定价值。Objective:To analyze the changes of serum membrane-bound mucin-1 of epithelial cells(MUC1)and CC-modified chemokine ligand 11(CCL11)levels before and after treatment of recurrent oral ulcer in children and their predictive value for recurrence.Methods:A total of 136 children with recurrent oral ulcer were taken as observation group,and 142 healthy patients without oral ulcers were taken as the control group.The indexes of children with recurrent oral ulcer before and after treatment were compared,and the indexes of non-recurrence group and recurrence group were compared.Serum levels of MUC1,CCL11,TNF-αand IL-2 were determined by ELISA.The correlation of serum MUC1,CCL11 with TNF-αand IL-2 was analyzed by Pearson method.ROC curve was used to evaluate the predictive value of serum MUC1 and CCL11 for recurrence of recurrent oral ulcer.Results:Compared with the control group,the serum levels of MUC1,CCL11,TNF-αand IL-2 in the observation group were obviously increased(all P<0.05).Serum levels of MUC1,CCL11,TNF-αand IL-2 were significantly decreased after treatment compared with those before treatment(all P<0.05).Pearson analysis showed that serum MUC1 and CCL11 were positively correlated with TNF-αand IL-2(all P<0.05).After treatment,compared with the non-recurrence group,the serum levels of MUC1,CCL11,TNF-αand IL-2 in the recurrence group were obviously increased(all P<0.05).The ROC curve analysis showed that the AUC of MUC1 and CCL11 for predicting the recurrence of recurrent oral ulcers was 0.886 and 0.813,respectively;the AUC of MUC1 combined with CCL11 for predicting the recurrence of recurrent oral ulcer was 0.908,with a sensitivity of 84.40%and a specificity of 86.50%.Conclusion:After treatment,the serum levels of MUC1 and CCL11 in children with recurrent oral ulcer are obviously decreased,which has a certain value in predicting the recurrence of recurrent oral ulcer in children.
