摘 要:目的:分析乙醛脱氢酶2基因(ALDH2)Glu504Lys多态性与脂肪肝之间的关系。方法:选取2018年1月—2019年12月在粤北人民医院体检中心体检人员共848例,分成脂肪肝组(n=275例)及对照组(n=573)。通过荧光定量PCR法检测ALDH2基因多态性。结果:对照组ALDH2 GG、GA和AA基因型所占比例分别为57.9%、37.0%和5.1%。脂肪肝组ALDH2 GG、GA和AA基因型所占比例分别为62.9%、33.5%和3.6%。两组ALDH2基因型分布比较,差异无统计学意义(P=0.326)。ALDH2 GG基因型组的收缩压、舒张压、甘油三酯、谷丙转氨酶、谷草转氨酶、γ-谷氨酰转肽酶、空腹血糖的血清学水平都高于GA/AA基因型组,差异有统计学意义(P<0.05)。Logistic回归分析显示,性别、年龄、总胆固醇、甘油三酯、低密度脂蛋白胆固醇、谷丙转氨酶、谷草转氨酶、γ-谷氨酰转肽酶、空腹血糖、尿酸是脂肪肝发生的危险因素(P<0.05),HDL-C是脂肪肝发生的保护因素(P<0.05)。结论:ALDH2基因Glu504Lys多态性与广东省韶关市人群脂肪肝发生无显著联系。Objective To investigate the association between Aldehyde Dehydrogenase 2(ALDH2)Glu504Lys and fatty liver.Methods In this study,a total of 848 subjects from the physical examination center of Yuebei People’s Hospital from January 2018 to December 2019 were enrolled and were divided into a fatty liver group(275 cases)and a control group(573 cases).The ALDH2 gene polymorphism was detected by fluorescence quantitative PCR.Results The proportions of ALDH2 GG,GA and AA genotypes in the control group were 57.9%,37.0%and 5.1%,respectively.The proportions of ALDH2 GG,GA and AA genotypes in fatty liver group were 62.9%,33.5%and 3.6%,respectively.There was no significant difference in the distribution of ALDH2 genotypes between the two groups(P=0.326).The systolic blood pressure,diastolic blood pressure,the serum levels of triglyceride,alanine aminotransferase,aspartate aminotransferase,γ-glutamyl transpeptidase,and fasting blood glucose in the ALDH2 GG genotype group were higher than those in the GA/AA gene type group(all P<0.05).Logistic regression analysis showed that male,age,total cholesterol,triglyceride,low-density lipoprotein cholesterol,alanine aminotransferase,aspartate aminotransferase,γ-glutamyl transpeptidase,fasting blood glucose,and uric acid were the risk factors for fatty liver.HDL-C is a protective factor for the occurrence of fatty liver.Conclusion The Glu504Lys polymorphism of ALDH2 gene has no clear relationship with the occurrence of fatty liver in the population of Shaoguan City,Guangdong Province.
