摘 要:目的通过分析儿童上前牙区多生牙患者的锥形束CT(cone beam CT,CBCT)影像学特点,探讨引起多生牙邻牙牙根吸收的危险因素及其临床应用价值。方法对166例上前牙区多生牙患者的CBCT进行观察统计,从多生牙在牙槽骨中的牙冠近远中倾斜方向、唇腭侧位置、垂直深度、多生牙的牙根发育及牙囊情况进行分析。由2名影像学专家用双盲法对CBCT产生的图像进行分析,采用非参数统计及二元Logistic回归解析。结果多生牙患者的性别与多生牙位置及牙囊大小对邻牙牙根吸收无明显相关性,而多生牙引起的邻牙牙根吸收可能与多生牙牙冠方向、牙根形成与否有关;多生牙牙长轴近中倾斜及牙囊增大的情况下,邻牙牙根吸收风险增加。结论根据多生牙牙长轴方向可以早期发现并预测牙根吸收的可能性。Objective To investigate the risk factors of impacted supernumerary teeth-induced adjacent teeth root absorption and predict the incidence of root resorption.Methods The CBCT of 166 patients with supernumerary teeth in the anterior region was observed and statistically analysed based on the crown mesiodistal direction,labial-palatal position,vertical depth,root development and follicle size of supernumerary teeth in alveolar bone.Nonparametric statistics and binary logistic regressions were used.Results Individual analysis of potential risk factors showed that crown mesiodistal direction and root developmental stage were associated significantly with root resorption of adjacent teeth.Binary logistic regression showed that supernumerary teeth with wide follicles and mesial inclination of crown increased the risk of the root absorption of its adjacent teeth.Conclusions The risk factors of root resorption can be detected and predicted early according to the long axis direction of supernumerary teeth.
