摘  要:目的 分析不同骨面型成年人上颌窦形态差异,探讨其与颅颌面骨骼形态之间的相关性。方法 将180例成人根据矢状骨面型分为骨性Ⅰ类、骨性Ⅱ类以及骨性Ⅲ类3组,再根据垂直骨面型将每组患者分为低角、均角以及高角3个亚组,所有患者均拍摄头颅侧位片及锥束计算机断层扫描成像(CBCT),测量并比较各组双侧上颌窦长度、宽度、高度以及体积,并对上颌窦大小(长、宽、高)、体积与颅颌面骨骼形态进行相关性分析。结果 3组间及亚组间患者的上颌窦长度、宽度、高度以及体积比较,差异均无统计学意义(P> 0.05);上颌窦长度与前颅底长(S-N)、上颌基骨长(Ptm-A)、面中长度(Co-A)、U1-NA距、上面高(N-ANS)呈正相关(P <0.05);上颌窦宽度与后颅底长(Ar-S)、Ptm-A、Co-A、N-ANS、全面高(N-Me)呈正相关,与S-Ptm呈负相关(P <0.05);上颌窦高度与SN-MP、下颌平面角(FH-MP)、Ar-S、Ptm-A、牙合平面角(OP-FH)、U1-ANS、U1-PP、U6-PP、N-ANS、下面高(ANS-Me)、N-Me、后面高(S-Go)、下颌升支高度(Go-Co)呈正相关,与S-Ptm呈负相关(P <0.05);上颌窦体积与S-N、Ar-S、Ptm-A、Co-A、U1-ANS、U1-PP、U6-PP、N-ANS、ANS-Me、N-Me、S-Go、Go-Co呈正相关,与S-Ptm呈负相关(P <0.05)。结论 不同骨面型成年人的上颌窦长度、宽度、高度以及体积无明显差异,上颌窦的大小、体积与颅底形态、垂直骨面型、上颌骨长度、下颌升支高度、上切牙和上磨牙的高度、面高均有相关关系。Objective To analyze the morphology difference of the maxillary sinus of adult patients with different skeletal patterns,and the correlation between maxillary sinus morphology and craniomaxillofacial bone morphology. Methods A total of 180 adult patients were divided into three groups according to sagittal skeletal patterns: classⅠ,classⅡ,and classⅢ. Then,each related group was divided into three sub-groups according to vertical skeletal patterns: low angle,normal angle,and high angle. All the related patients were detected by lateral cephalometric radiographs and cone beam computed tomography(CBCT) for cephalometric analysis,and bilateral maxillary sinus length,width,height and volume were measured and compared. The correlational study of maxillary sizes(length,width,and height) and volume and cranio-maxillofacial bone morphology were analyzed. Results There were no significant differences in length,width,height and volume of the maxillary sinus among the three groups and sub-groups(P > 0. 05). The length of maxillary sinus was positively correlated with S-N,Ptm-A,Co-A,U1-NA,N-ANS(P < 0. 05);the width of maxillary sinus was positively correlated with Ar-S,Ptm-A,Co-A,N-ANS,N-Me,and negatively correlated with S-Ptm(P <0. 05). The height of maxillary sinus was positively correlated with SN-MP,FH-MP,Ar-S,Ptm-A,OP-FH, U1-ANS, U1-PP, U6-PP, N-ANS, ANS-Me, N-Me, S-Go, Go-Co, and negatively correlated with S-Ptm(P < 0. 05). The volume of maxillary sinus was positively correlated with S-N,Ar-S,Ptm-A,Co-A,U1-ANS,U1-PP,U6-PP,N-ANS,ANS-Me,N-Me,S-Go and Go-Co,and negatively correlated with S-Ptm.(P < 0. 05). Conclusion There are no differences in the length,width,height,and volume of the maxillary sinus in the adult patients with different skeletal patterns.The size and volume of maxillary sinus are related to the morphology of the skull base,the vertical skeletal pattern,the length of maxilla,the height of mandibular ramus,the height of the upper incisors and upper molars,and the facial height.
