19665例人乳头瘤病毒感染类别、多重感染和递减顺序分析

摘　　要：目的分析大样本机会性宫颈感染人乳头瘤病毒(human papilloma virus,HPV)的现状、基因亚型、感染递减顺序、多重亚型感染和递减顺序等。方法收集2010年1月1日—2021年12月30日淮安市妇幼保健院妇科门诊检查的HPV标本21992例,采用实时荧光PCR技术定性检测,剔除检查结果重复的标本后共19665份样本。研究分析HPV感染总检出率、各亚型分布、感染递减顺序、多重亚型感染及递减顺序特征等。结果HPV高危型检出率是21.15%,高危亚型感染率递减顺序HPV52、HPV16、HPV58、HPV53、HPV39、HPV51、HPV18、HPV33、HPV31、HPV56、HPV59、HPV68、HPV45、HPV66、HPV35、HPV73、HPV82。HPV高危型亚型多重感染率递减顺序是HPV18、HPV16、HPV51、HPV31、HPV33、HPV73、HPV52、HPV58、HPV39、HPV53、HPV59、HPV56、HPV66、HPV35、HPV82、HPV45和HPV68。HPV低危亚型感染率由高至低顺序为HPV81、HPV42、HPV43、HPV44、HPV6、HPV11、HPV53。结论本研究妇女机会性高危型HPV亚型感染率前6位是HPV52、HPV16、HPV58、HPV53、HPV39、HPV51,后5位是HPV45、HPV66、HPV35、HPV73、HPV82。高危HPV感染以单一亚型为主,2种亚型也占一定比例。低危亚型HPV6、HPV11分别占7种低危亚型的第5和6位。本研究对区域性HPV疫苗型别选择以及HPV感染精准防治有指导价值。Objective To investigate the current condition,gene subtypes,decreasing order of opportunistic cervical infection with human papilloma virus(HPV),and multiple-subtype infection and decreasing order in large samples.Methods A total of 21992 female patients who were admitted to Department of Gynecology,the Maternal and Child Health Hospital of Huai′an from January 1,2010 to December 30,2020 were enrolled.Repeated HPV samples were deleted using reql-time flurescence PCR,resulting in 19665 samples.The total HPV infection detection rate,the distribution of each subtype,infection descreasing order,multiple subtype infection and decreasing charateristics were analyzed.Results The detection rate of high-risk HPV subtype was 21.15%.According to the the decreasing order of infection rate,the high-risk subtypes were listed as follows:HPV52,HPV16,HPV58,HPV53,HPV39,HPV51,HPV18,HPV33,HPV31,HPV56,HPV59,HPV68,HPV45,HPV66,HPV35,HPV73 and HPV82.According to the decreasing order of multiple infections,the high-risk HPV subtypes were listed as follows:HPV18,HPV16,HPV51,HPV31,HPV33,HPV73,HPV52,HPV58,HPV39,HPV53,HPV59,HPV56,HPV66,HPV35,HPV82,HPV45 and HPV68.According to the decreasing order of infection,the low-risk HPV subtypes were listed as follows:HPV81,HPV42,HPV43,HPV44,HPV6,HPV11 and HPV53.Conclusions The top six high-risk subtypes of opportunistic HPV infection are HPV52,HPV16,HPV58,HPV53,HPV39 and HPV51,whereas the last five high-risk subtypes are HPV45,HPV66,HPV35,HPV73 and HPV82.Single high-risk subtype HPV infection is predominant,while double high-risk subtypes of HPV infection exist to some extent.The low-risk subtypes HPV 6 and HPV11 ranked the fifth and sixth among seven low-risk subtypes.These findings provide guidance for reginal selection of HPV vaccine and precise prevention of HPV infection.
