亚低温联合鼻内镜下鼻颅底肿瘤切除术治疗前颅底肿瘤的临床疗效及安全性分析

摘　　要：目的探讨亚低温联合鼻内镜下鼻颅底肿瘤切除术治疗前颅底肿瘤的临床疗效及安全性分析。方法回顾性分析2018年1月—2020年12月郑州某医院(50例)与河南某医院(30例)颅底外科收治的80例良性前颅底肿瘤手术患者的临床资料,按照治疗措施分为对照组(39例接受鼻内镜下鼻颅底肿瘤切除术)和研究组(41例接受亚低温联合鼻内镜下鼻颅底肿瘤切除术)两组,比较两组临床疗效。结果研究组术中失血量、卧床时间、住院时间、术后总并发症发生率等均少于对照组(P<0.05);术后研究组认知功能(MMSE)评分高于对照组(P<0.05);术后研究组D-二聚体(D-D)、血小板平均聚集率(Ara)及凝血酶原时间(PT)等均优于对照组(P<0.05)。结论联合亚低温技术实施鼻颅底肿瘤切除手术可有效促进前颅底肿瘤术后患者认知功能的恢复,提高患者血液循环稳定性,降低术后并发症发生风险。Objective To explore the clinical efficacy and safety of sub-hypothermia combined with nasal endoscopic resection of anterior skull base tumors.Methods A total of 80 patients suffered with anterior skull base tumors those were admitted to skull base surgery department of a hospital of Zhengzhou(50 cases)and a hospital of Henan province(30 cases)during January 2018 and December 2020 were analyzed retrospectively.According to the treatment measures they were divided into control group(39 cases,accept nasal endoscopic tumor resection),study group(41 cases,sub-hypothermia combined with nasal endoscopic tumor resection),the clinical effect of two groups were compared.Results Intraoperative blood loss,time stay in bed,hospital stay,and total postoperative complications in the study group were less than those in the control group(P<0.05).Postoperative cognitive function score(MMSE)in the study group was higher than that in the control group(P<0.05).After the operation,the concentration of D-dimer(D-D),mean platelet aggregation(Ara)and prothrombin time(PT)of the study group were superior to the control group(P<0.05).Conclusions Combined with mild hypothermia,the resection of nasal skull base tumor could effectively promote the recovery of cognitive function,improve the stability of blood circulation and reduce the risk of postoperative complications in patients with anterior skull base tumor.
