摘 要:目的探讨糖尿病患者白内障超声乳化术后早期角膜内皮变化及眼底病变程度与术后疗效的相关性。方法选取2018年1月至2020年12月于我院接受超声乳化手术的60例糖尿病合并白内障患者(观察组)与60例非糖尿病白内障患者(对照组)进行回顾性研究。比较患者的最佳矫正视力、视网膜厚度、角膜内皮指标,分析视网膜厚度、角膜内皮指标与最佳矫正视力的相关性。结果手术后1周,观察组的最佳矫正视力低于对照组,中央视网膜厚度、黄斑区视网膜厚度均大于对照组(P<0.05)。手术后1周,观察组的角膜内皮细胞密度、六角形细胞百分比均低于对照组,面积、变异系数均大于对照组(P<0.05)。手术后1周,增殖期的最佳矫正视力低于非增殖期,中央视网膜厚度、黄斑区视网膜厚度大于非增殖期(P<0.05)。手术后1周,增殖期的角膜内皮细胞密度、六角形细胞百分比均低于非增殖期,角膜内皮细胞面积、变异系数均大于非增殖期(P<0.05)。Spearman相关性分析结果显示,中央视网膜厚度、黄斑区视网膜厚度、角膜内皮细胞面积、变异系数与糖尿病合并白内障患者术后最佳矫正视力呈负相关,角膜内皮细胞密度、六角形细胞百分比与糖尿病合并白内障患者术后最佳矫正视力呈正相关(P<0.05)。结论糖尿病合并白内障患者经超声乳化手术治疗后的角膜内皮损伤更加严重,其视力改善效果与眼底病变程度有关。Objective To investigate the correlation between early changes of corneal endothelium,fundus lesions and postoperative efficacy in diabetes mellitus patients after phacoemulsification.Methods From January 2018 to December2020,60 patients with diabetes mellitus complicated with cataract(observation group)and 60 patients with non-diabetes mellitus complicated with cataract(control group)who underwent phacoemulsification in our hospital were selected for retrospective study.The best corrected visual acuity,retinal thickness and corneal endothelial indexes of the patients were compared,the correlation between retinal thickness,corneal endothelial indexes and best corrected visual acuity were analyzed.Results One week after operation,the best corrected visual acuity in the observation group was lower than that in the control group,and the central retinal thickness and macular retinal thickness of the observation group were greater than those in the control group(P<0.05).One week after operation,the corneal endothelial cell density and percentage of hexagonal cells in the observation group were lower than those in the control group,and the area and coefficient of variation were greater than those in the control group(P<0.05).One week after operation,the best corrected visual acuity in the proliferative period was lower than that in the non-proliferative period,and the central retinal thickness and macular retinal thickness were greater than those in the non-proliferative period(P<0.05).One week after operation,the corneal endothelial cell density and percentage of hexagonal cells in the proliferative period were lower than those in the non-proliferative period,and the corneal endothelial cell area and coefficient of variation were greater than those in the non-proliferative period(P<0.05).Spearman correlation analysis results showed that central retinal thickness,macular retinal thickness,corneal endothelial cell area and coefficient of variation were negatively correlated with best corrected visual acuity in patients
