引导组织再生联合牙内骨种植治疗老年中重度牙周病的效果观察

摘　　要：目的:探讨引导组织再生(GTR)联合牙内骨种植治疗老年中重度牙周病的临床效果。方法:回顾性分析2021年1-6月广州市中西医结合医院收治的86例老年中重度牙周病患者,根据治疗方案差异分为观察组与对照组,对照组43例单纯接受GTR治疗,观察组43例采取GTR联合牙内骨种植治疗,对比两组牙周病治疗效果及牙周功能情况。结果:治疗前两组探诊附着水平(PAL)、探诊深度(PD)比较,差异无统计学意义(P>0.05)。治疗后,观察组PAL(5.82±0.44)mm、PD(3.20±0.52)mm明显低于对照组[(7.42±0.79)mm、(5.87±0.45)mm](P<0.05)。治疗后观察组总有效率为95.35%,明显高于对照组治疗的72.09%(χ^(2)=9.318,P<0.05)。治疗后,观察组口腔健康生活质量评价指数(GOHAI)各维度评分明显低于对照组(P<0.05)。观察组不良反应发生率为4.65%,明显低于对照组的18.60%(χ^(2)=4.329,P<0.05)。结论:采用引导组织再生联合牙内骨种植方案治疗老年中重度牙周病可有效改善患者牙周功能及口腔健康状态,对提升老年中重度牙周病治疗有效率及患者生活质量具有积极作用。Objective:To investigate the clinical efficacy of guided tissue regeneration (GTR) combined with endodontic bone implantation in the treatment of moderate and severe periodontal disease in the elderly.Method:A total of 86 elderly patients with moderate and severe periodontal disease admitted to Guangzhou Hospital of Integrated Traditional Chinese and Western Medicine from January to June 2021,and they were divided into observation group and control group according to the difference of treatment plan,43 patients in the control group were treated with GTR alone,and 43 patients in the observation group were treated with GTR combined with endodontic bone implantation.The periodontal disease treatment effect and periodontal function of the two groups were compared.Result:There were no significant differences in probe attachment level (PAL) and probe depth (PD) between the two groups before treatment (P>0.05).After treatment,the PAL (5.82±0.44) mm and PD (3.20±0.52) mm in the observation group were significantly lower than those in the control group[(7.42±0.79) mm,(5.87±0.45) mm](P<0.05).After treatment,the total effective rate in the observation group was 95.35%,which was significantly higher than 72.09% in the control group (χ^(2)=9.318,P<0.05).After treatment,the scores of each dimension of geriatric oral health assessment index (GOHAI) in the observation group were significantly lower than those in the control group (P<0.05).The incidence of adverse reactions in the observation group was only 4.65%,which was significantly lower than 18.60% in the control group (χ^(2)=4.329,P<0.05).Conclusion:Guided tissue regeneration combined with endodontic bone implantation in the treatment of moderate and severe periodontal disease in the elderly can effectively improve the periodontal function and oral health status of patients,and has a positive effect on improving the treatment efficiency and quality of life of elderly patients with moderate and severe periodontal disease.
