内镜下精准食管胃静脉曲张断流术后再出血的危险因素分析

摘　　要：目的:探讨内镜下精准食管胃静脉曲张断流术(ESVD)后再出血的危险因素。方法:根据方便抽样方式选取2020年2月-2021年12月龙岩市第二医院消化内科接受ESVD治疗的84例食管胃静脉曲张破裂出血(EVB)患者。收集患者一般资料。统计所有患者ESVD术后再出血情况,并对ESVD术后再出血进行单因素及多因素分析。结果:84例患者ESVD术后再出血39例(46.43%),无再出血45例(53.57%)。单因素分析显示,再出血患者年龄≥60岁、门静脉血栓、腹腔积液>120 ml、红色征、血肌酐>186 μmol/L、凝血酶时间>20 s占比均高于无再出血患者(P<0.05)。多因素分析显示,年龄≥60岁、门静脉血栓、腹腔积液>120 ml、红色征、血肌酐>186 μmol/L、凝血酶时间>20 s均是ESVD术后再出血的高危因素(P<0.05)。结论:ESVD术后再出血发生与年龄、门静脉血栓、腹腔积液、红色征、血肌酐、凝血酶时间有关。Objective:To investigate the risk factors of rebleeding after endoscopic precision esophagogastric variceal devascularization (ESVD).Method:A total of 84 patients with esophagogastric variceal hemorrhage (EVB) who received ESVD treatment in the Department of Gastroenterology,the Second Hospital of Longyan City from February 2020 to December 2021 were selected according to the convenient sampling method.General data of patients were collected.Rebleeding after ESVD was recorded in all patients.The rebleeding after ESVD was analyzed by univariate and multivariate analysis.Result:Among 84 patients,39 cases (46.43%) had rebleeding after ESVD,and 45 cases (53.57%) had no rebleeding.Univariate analysis showed that the proportion of age ≥60 years old,portal vein thrombosis,abdominal effusion >120 ml,red sign,serum creatinine >186 μmol/L,thrombin time >20 s in patients with rebleeding were higher than those in patients without rebleeding (P<0.05).Multivariate analysis showed that age ≥60 years old,portal vein thrombosis,abdominal effusion >120 ml,red sign,serum creatinine >186 μmol/L,thrombin time >20 s were all risk factors for rebleeding after ESVD (P<0.05).Conclusion:The incidence of rebleeding after ESVD is related to age,portal vein thrombosis,abdominal effusion,red sign,serum creatinine and thrombin time.
