摘 要:目的探讨高分辨率CT对肺小结节及早期肺癌的鉴别诊断价值。方法根据CT检查方式的不同将126例肺小结节及早期肺癌患者分为常规CT组和高分辨率CT组,每组63例。比较两种扫描方式的肿瘤征象显示情况、扫描图像质量以及两种方式对早期肺癌的诊断效能。结果高分辨率CT组毛刺征、分叶征、棘突征、钙化征、空泡征、支气管征、胸膜凹陷征及血管集束征的显示率均高于常规CT组,差异均有统计学意义(P﹤0.05)。高分辨率CT组的CT扫描图像优良率为95.24%,明显高于常规CT组的71.43%(P﹤0.01)。高分辨率CT诊断早期肺癌的灵敏度、特异度、准确度分别为94.59%(35/37)、92.31%(24/26)、93.65%(59/63),均高于常规CT的75.00%(30/40)、78.26%(18/23)、76.19%(48/63)。结论高分辨率CT可清晰显示肿瘤病灶的形态学特征和内部结构,其图像质量较好,对肺小结节及早期肺癌的鉴别诊断价值高于常规CT。Objective To investigate the differential diagnostic value of high-resolution CT in small pulmonary nodules and early lung cancer. Method A total of 126 patients with small pulmonary nodules and early lung cancer were divided into conventional CT group and high-resolution CT group according to different CT examination methods, with 63cases in each group. The tumor sign display, scanning image quality, and the diagnostic efficiency for early lung cancer were compared between the two scanning methods. Result The display rates of spicule sign, lobulation sign, spinous process sign, calcification sign, vacuole sign, bronchial sign, pleural depression sign, and vascular bundle sign in the highresolution CT group were higher than those in the conventional CT group, and the differences were statistically significant(P<0.05). The excellent and good rate of CT scanning images in the high-resolution CT group was 95.24%, which was significantly higher than 71.43% in the conventional CT group(P<0.01). The sensitivity, specificity, and accuracy of high-resolution CT in the diagnosis of early lung cancer were 94.59%(35/37), 92.31%(24/26), and 93.65%(59/63), respectively, which were higher than 75.00%(30/40), 78.26%(18/23), 76.19%(48/63) of conventional CT. Conclusion High-resolution CT could clearly display the morphological features and internal structure of tumor lesions, the image quality is better, and the differential diagnostic value of small pulmonary nodules and early lung cancer is higher than conventional CT.
