龈上洁治术与牙周非手术治疗单纯中重度牙周炎患者的临床效果对比

摘　　要：目的:研究比较龈上洁治术与牙周非手术治疗单纯中重度牙周炎患者的临床效果。方法:选择2019年1月-2021年1月福建医科大学附属龙岩第一医院口腔科门诊收治的140例单纯中重度牙周炎患者作为研究对象,采用随机数字表法分为试验组(n=70)和对照组(n=70)。试验组采用牙周非手术治疗,对照组采用龈上洁治术治疗。比较两组治疗前后牙周指标、炎症因子指标、血管内皮功能指标。结果:治疗后1个月,两组临床附着丧失(CAL)、探诊深度(PD)、探诊出血指数(BOP)及血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-1β(IL-1β)水平均低于治疗前,且试验组均低于对照组,差异均有统计学意义(P<0.05)。试验组治疗后1个月肱踝脉搏波传导速度(baPWV)、可溶性细胞间黏附分子(sICAM-1)水平均低于治疗前与对照组,差异均有统计学意义(P<0.05)。结论:牙周非手术治疗单纯中重度牙周炎相较龈上洁治术能更加有效地改善患者牙周健康,对降低炎症因子水平、改善血管内皮功能亦有积极作用,值得临床推广。Objective:To study and compare the clinical effect of supragingival scaling and periodontal non-surgical treatment for patients with simple moderate and severe periodontitis.Method:A total of 140 patients with simple moderate and severe periodontitis treated in the Outpatient Department of Stomatology of Longyan First Hospital Affiliated to Fujian Medical University from January 2019 to January 2021 were selected as the research objects.They were divided into the experimental group (n=70) and the control group (n=70) by random number table method.The experimental group was treated with periodontal non-surgical treatment,and the control group was treated with supragingival scaling.The periodontal indexes,inflammatory factor indexes and vascular endothelial function indexes were compared between two groups before and after treatment.Result:At 1 month after treatment,the clinical attachment loss (CAL),probing depth (PD),probing bleeding index (BOP) and serum tumor necrosis factor-α (TNF-α),interleukin-6 (IL-6),interleukin-1β (IL-1β) levels in two groups were lower than those before treatment,and the indexes in the experimental group were lower than those of the control group,the differences were statistically significant (P<0.05).The brachial-ankle pulse wave velocity (baPWV) and soluble intercellular adhesion molecule (sICAM-1) levels in the experimental group at 1 month after treatment were lower than those before treatment and the control group,and the differences were statistically significant (P<0.05).Conclusion:Compared with supragingival scaling,periodontal non-surgical treatment of simple moderate and severe periodontitis can more effectively improve patients’ periodontal health,reduce the level of inflammatory factors and improve vascular endothelial function,which is worthy of clinical promotion.
