DC-CIK疗法联合伽马刀治疗局部淋巴结转移性食管癌患者的疗效

摘　　要：目的研究树突状细胞与细胞因子诱导的杀伤细胞(DC-CIK)疗法联合伽马刀治疗局部淋巴结转移性食管癌(esophageal cancer,EC)患者的疗效及对免疫功能的影响。方法以南阳市第一人民医院2018年2月至2019年10月收治的108例局部淋巴结转移性EC患者为研究对象,采用随机数字表分为对照组、联合组,每组各54例。对照组采用伽马刀治疗,联合组采用DC-CIK联合伽马刀治疗。比较两组临床疗效、治疗前后免疫功能指标(CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+))、血清血小板反应蛋白-1(TSP-1)、基质金属蛋白酶-9(MMP-9)、血管内皮生长因子(VEGF)、钙结合蛋白(S100A4)水平、1年及2年生存率。结果联合组疼痛、器官受侵或受压变形、声音嘶哑、进食梗阻缓解率高于对照组(P<0.05);治疗2个周期后联合组CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平明显高于对照组(P<0.05);治疗2个周期后联合组血清TSP-1水平高于对照组,MMP-9、VEGF、S100A4水平低于对照组(P<0.05);随访2年,联合组1年生存率(67.35%)、2年生存率(30.61%)高于对照组(42.00%,12.00%),P<0.05。结论采用DC-CIK疗法与伽马刀联合治疗局部淋巴结转移性EC患者,可获得更高临床疗效,增强机体免疫应答反应,进一步杀死癌细胞,抑制肿瘤细胞转移,缓解临床症状,对延长患者生存期具有重要作用。Objective To study the effect of dendritic cells and cytokine induced killer cells(DC-CIK)combined with Gamma knife in the treatment of patients with local lymph node metastatic esophageal cancer(EC)and its influence on immune function.Methods A total of 108 patients with local lymph node metastatic EC admitted to Nanyang First People's Hospital from February 2018 to October 2019 were selected as the research objects.They were randomly divided into control group and combined group,with 54 cases in each group.The control group was treated with Gamma knife,and the combined group was treated with DC-CIK combined with Gamma knife.The clinical efficacy,immune function indexes(CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)),serum thrombospondin-1(TSP-1),matrix metalloproteinase-9(MMP-9),vascular endothelial growth factor(VEGF),calcium-binding protein(S100A4)levels before and after treatment,and 1-year and 2-year survival rates were compared between the two groups.Results The remission rates of pain,organ invasion or compression deformation,hoarseness and eating obstruction in the combined group were higher than those in the control group(P<0.05);After 2 cycles of treatment,the levels of CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)in the combined group were significantly higher than those in the control group(P<0.05);After 2 cycles of treatment,the serum TSP-1 level in the combined group was higher than that in the control group,while the levels of MMP-9,VEGF,S100A4 were lower than those in the control group(P<0.05);After 2-year follow-up,the 1-year survival rate (67.35%)and 2-year survival rate(30.61%)of the combined group were higher than those of the control group(42.00%,12.00%),P<0.05.Conclusion The combination of DC-CIK therapy and Gamma knife therapy for patients with local lymph node metastatic EC can achieve higher clinical efficacy,enhance the immune response of the body,further kill cancer cells,inhibit tumor cell metastasis,alleviate clinical symptoms,and plays an important role in prolonging the survival
