摘 要:目的分析同型半胱氨酸(Hcy)、神经特异性烯醇化酶(NSE)水平对脑出血患者认知功能障碍的评估价值。方法选取2019年1月-2021年1月我院收治的66例脑出血患者作为研究对象,以简易精神状态检查量表(MMSE)评分为依据,分为认知功能障碍组(MMSE≤24分)与认知功能正常组(MMSE>24分),每组33例。比较两组血清Hcy、NSE水平,并对其Hcy、NSE水平与认知功能障碍程度的相关性进行分析。结果认知功能障碍组血清Hcy、NSE水平高于认知功能正常组(P<0.05);不同认知功能障碍患者血清Hcy、NSE水平比较,差异有统计学意义(P<0.05),其中血清Hcy、NSE水平由高至低依次为高度认知障碍患者>中度认知障碍患者>轻度认知障碍患者;不同血清Hcy、NSE水平患者MMSE、MoCA评分比较,差异有统计学意义(P<0.05),其中MMSE、MoCA评分由高至低依次为重度组>轻度组>正常组;Pearson相关性分析显示,脑出血患者血清Hcy、NSE水平与认知障碍呈正相关(r=0.632、0.714,P=0.003、0.000)。结论脑出血患者血清Hcy、NSE水平可反映认知功能障碍的严重程度,对脑出血患者认知功能障碍具有重要评估价值。Objective To analyze the value of homocysteine(Hcy)and neuron-specific enolase(NSE)levels in evaluating cognitive dysfunction in patients with cerebral hemorrhage.Methods A total of 66 patients with cerebral hemorrhage admitted to our hospital from January 2019 to January 2021 were selected as the study subjects.Based on the Mini-Mental State Examination(MMSE)score,they were divided into cognitive dysfunction group(MMSE≤24 scores)and normal cognitive function group(MMSE>24 scores),with 33 cases in each group.The levels of serum Hcy and NSE were compared between the two groups,and the correlation between the levels of Hcy and NSE and the degree of cognitive dysfunction was analyzed.Results The levels of serum Hcy and NSE in the cognitive impairment group were higher than those in the normal cognitive function group(P<0.05).There were statistically significant differences in serum Hcy and NSE levels in patients with different cognitive impairment(P<0.05),and the levels of serum Hcy and NSE from high to low were high cognitive impairment>moderate cognitive impairment>mild cognitive impairment.There were significant differences in MMSE and MoCA scores among patients with different serum levels of Hcy and NSE(P<0.05),and the MMSE and MoCA scores from high to low were severe group>mild group>normal group.Pearson correlation analysis showed that serum Hcy and NSE levels were positively correlated with cognitive impairment in patients with ICH(r=0.632,0.714,P=0.003,0.000).Conclusion The serum levels of Hcy and NSE in patients with intracerebral hemorrhage can reflect the severity of cognitive dysfunction,and have important evaluation value for cognitive dysfunction in patients with intracerebral hemorrhage.
