摘　　要：目的:观察FOCUS-PDCA程序在降低经肠内营养管给药堵管发生率中的效果。方法:选取2019年1月—2020年11月浙江省宁波市第一医院ICU肠内营养结束后,经肠内营养管给药的患者219例,分为对照组110例(2019年1月—7月实施FOCUS-PDCA程序前)和观察组109例(2020年5月—11月实施FOCUS-PDCA程序后),运用FOCUS-PDCA程序评估经肠内营养管给药堵管的原因,制定经肠内营养管给药的规范化措施,对实施前后肠内营养管药物堵管的发生状况进行分析。结果:分别比较两组中,三种不同的肠内营养管药物堵管发生率,实施FOCUS-PDCA程序后,观察组鼻肠管堵管3例(6.00%),三腔喂养管堵管2例(5.26%),PEJ管堵管2例(9.52%);对照组鼻肠管堵管10例(19.23%),三腔喂养管堵管8例(21.05%),PET管堵管7例(35.00%)。观察组均低于对照组,差异有统计学意义(P<0.05),对照组110例和观察组109例肠内营养管药物堵管比较,观察组低于对照组,差异有统计学意义(P<0.01)。结论:运用FOCUS-PDCA程序可以有效地降低患者经肠内营养管给药堵管的发生率,值得临床应用。Objective To study the effect of FOCUS-PDCA program in reducing the incidence of enteral nutrition tube blockage.Methods 219 patients after enteral nutrition in ICU of the hospital from Jan.2019 to Nov.2020.They were divided into control group(110 cases January-July 2019 Before the implementation of FOCUS-PDCA)and observation group(109 cases May-November 2020 After the implementation of FOCUS-PDCA).The FOCUS-PDCA program was used to evaluate the causes of drug blockage in enteral nutrition tube,formulate standardized measures for enteral nutrition tube administration,and analyze the occurrence of drug blockage in enteral nutrition tube before and after implementation.Results The incidence of three different types of enteral nutrition tube drug blockage in the two groups was compared.After the implementation of FOCUS-PDCA observation and control group nasointestinal tube blockage in 3 cases(6.00%)and 10 cases(19.23%)respectively,three cavity feeding tube blockage in 2 cases(5.26%),and 8 cases(21.05%)respectively PEJ tube blockage in 2 cases(9.52%)and 7 cases(35.00%)respectively the observation group was lower than the control group(P<0.05),110 cases in the control group and 109 cases in the observation group,the observation group was lower than the control group(P<0.01).Conclusion The FOCUS-PDCA program can effectively reduce the incidence of enteral nutrition tube blockage in patients,which is worthy of clinical application and promotion.
