摘  要:目的:从炎症、肝纤维化、肝代偿功能多个维度评估患者的肝功能,并探讨其与CHB患者HBV-DNA载量的关系。方法:设计回顾性研究方案,共纳入CHB病例80例,在予以血清HBV-DNA定量检测的基础上将其分为DL组(低量组)58例和GL组(高量组)22例,分别对比DL组和GL组患者的肝功能相关指标[包括肝功能常规三项(TBil、ALT、AST)、血清炎症反应指标(IL-2、IL-6、IL-17)、血清肝纤维化指标(HA、PCⅢ、Ⅳ-C)及肝功能代偿状态]。结果:GL组患者血清肝功能常规三项指标TBil、ALT、AST均高于DL组(P<0.05),GL组患者血清炎症反应指标IL-2、IL-6、IL-17均高于DL组(P<0.05),GL组患者血清肝纤维化指标HA、PCⅢ、Ⅳ-C均高于DL组(P<0.05)。GL组患者肝功能代偿中重度不全者比例为13.64%,高于DL组的0(P<0.05);GL组患者肝功能代偿正常者比例为45.45%,低于DL组的68.97%(P<0.05)。结论:随CHB患者HBV-DNA载量的增加,患者肝细胞炎性损伤加重,肝纤维化程度加重,肝功能代偿不全加剧,并在其共同作用下造成患者肝功能下降。Objective: To evaluate the liver function of patients from multiple dimensions of inflammation, liver fibrosis and liver compensation function, and to explore the correlation mechanism between the liver function and the amount of HBV in CHB patients. Methods: A retrospective study was designed. 80 cases of CHB were Included in the DL group(low dose group, n = 58) and GL group(high dose group, n = 22) based on the quantitative detection of serum HBV-DNA. The(TBil, ALT, AST), serum inflammatory response indexes(IL-2, IL-6, IL-17), serum liver fibrosis indexes(HA, PCⅢ, Ⅳ-C), and liver function compensation status were compared between DL group and GL group. Results:TBil, ALT and AST in GL group were higher than those in DL group(P<0.05), and IL-2, IL-6 and IL-17 in GL group were higher than those in DL group(P<0.05). The levels of HA, PCⅢ and Ⅳ-C in GL group were higher than those in DL group(P<0.05). In the moderate-severe incomplete rate of liver function compensation, GL group and DL group were 13.64% and 0, respectively, P<0.05;The normal rate of liver function compensation was 45.45%, which was lower than 68.97% in DL group(P<0.05). Conclusion: When the HBV-DNA value increased, the inflammatory damage of hepatocytes in patients is aggravated, the degree of liver fibrosis is aggravated, and the compensation of liver function is aggravated, and the combined effect of these two factors causes the decline of liver function in patients.
