人参止血汤治疗急性非静脉曲张性上消化道出血(脾不统血证)的临床研究

摘　　要：目的 观察人参止血汤治疗急性非静脉曲张性上消化道出血(ANVUGIB)(脾不统血证)的临床疗效。方法 选取60例ANVUGIB(脾不统血证)患者,随机分为对照组与观察组,对照组给予常规治疗,观察组在给予常规治疗的同时加用人参止血汤,两组疗效均为7 d。统计两组患者的年龄、性别、入院时的血红蛋白量、Rockall评分,治疗后黑便天数、大便潜血天数、住院时间、凝血功能及治疗后血红蛋白量,治疗前后中医证候评分。结果 观察组大便潜血≤3 d及大便潜血<7 d的人数均明显多于对照组(P<0.05)。观察组黑便天数短于对照组(P<0.05)。观察组平均住院天数、血红蛋白量与对照组相近(P>0.05)。治疗后两组中医证候评分较前均呈下降趋势,且观察组胃脘痛、食欲减退的证候评分显著低于对照组(P<0.05)。结论 人参止血汤可显著缩短ANVUGIB(脾不统血证)患者的黑便及大便潜血时间,能够改善临床症状,降低中医证候评分。Objective:To observe the clinical efficacy of Renshen Zhixue Decoction in the treatment of acute non-varicose upper gastrointestinal bleeding(ANVUGIB). Methods:A total of 60 patients with ANVUGIB(syndrome of spleen failing to manage blood)were randomly selected and divided into control group and observation group. The control group was given conventional treatment,while the observation group was given conventional treatment and Renshen Zhixue Decoction. Both groups were observed for 7 days. The age,gender,hemoglobin amount and Rockall score of patients in the two groups at admission were counted. After treatment,the number of days in melena,fecal occult blood,hospital stay,coagulation function and hemoglobin content were counted.Results:The number of patients with fecal occult blood ≤3 days and fecal occult blood< 7 days in the observation group were significantly more than those in the control group(P >0.05). The days of melena in the observation group were shorter than those in the control group,and the difference was statistically significant(P< 0.05).The average length of hospital stay and hemoglobin in the observation group were similar to those in the control group(P >0.05). After treatment,the TCM syndrome scores of the two groups showed a downward trend,and the syndrome scores of epigastric pain and appetite loss in the observation group were significantly lower than those in the control group(P< 0.05). Conclusion:Renshen Zhixue Decoction can significantly shorten the time of melena and fecal occult blood in patients with ANVUGIB(syndrome of spleen failing to manage blood),improve the clinical symptoms,and reduce the TCM syndrome score.
