颅内胶质瘤患者血清中微小RNA-34/194表达与患者生存期的相关性

摘　　要：目的探究颅内胶质瘤患者血清中微小RNA-34/194表达与患者生存期的相关性。方法选取92例颅内胶质瘤患者作为研究组(n=92),同时选取64例行颅内减压术的非恶性肿瘤患者为对照组(n=64)。采集研究组手术前后及对照组清晨空腹静脉血9 ml,离心15 min,3000 r/min,使用实时荧光定量PCR法检测血清中miR-34和miR-194的表达水平。比较2组miR-34、miR-194表达差异,分析miR-34、miR-194表达与颅内胶质瘤患者病情程度和预后生存的相关性。结果研究组术前血清中miR-34、miR-194均比对照组低(P<0.05),术后水平均上升(P<0.05),但仍低于对照组(P<0.05)。比较不同级别脑胶质瘤患者手术后的miR-34、miR-194的表达水平可以得到,在Ⅰ级患者中均为高表达;Ⅱ级中miR-34高表达有29例,miR-194高表达有28例;Ⅲ级中miR-34高表达有2例,miR-194高表达有4例;Ⅳ级中均为低表达。通过为期3年的随访发现,miR-34高表达组患者生存率均高于低表达组(P<0.05);miR-194高表达组患者生存率均高于低表达组(P<0.05)。经ROC曲线可得,miR-34、miR-194预测患者存活期的AUC分别为0.809、0.764,截断值分别为2.11、2.05,两者联合预测的AUC为0.882。结论颅内胶质瘤患者病情越严重,其血清中miR-34、miR-194的表达水平越低;且患者术后的miR-34、miR-194的水平越高,存活期越长。ROC曲线显示:miR-34、miR-194的表达水平可以预测患者的生存期,且联合预测价值更高。Objective To explore the correlation between the expression of microrna-34/194 in the serum of patients with intracranial glioma and the survival time of patients.Methods 92 patients with intracranial gliomas were selected as the study group(n=92),and 64 patients with non malignant tumors underwent intracranial decompression were selected as the control group(n=64).9 ml of fasting venous blood was collected before and after operation in the study group and in the morning in the control group,centrifuged for 15 min and 3000 r/min,and the expression levels of miR-34 and miR-194 in serum were detected by real-time fluorescent quantitative PCR.The expression difference of miR-34 and miR-194 between the 2 groups was compared,and the correlation between the expression of miR-34 and miR-194 and the severity of the disease and the prognosis of patients with intracranial glioma was analyzed.Results The serum levels of miR-34 and miR-194 in the study group before operation were lower than those in the control group(P<0.05),and increased after operation(P<0.05),but still lower than those in the control group(P<0.05).Comparing the expression levels of miR-34 and miR-194 in patients with different grades of gliomas after surgery,we can find that they were all highly expressed in patients with grade I;There were 29 cases with high expression of miR-34 and 28 cases with high expression of miR-194 in grade II;In grade III,there were 2 cases with high expression of miR-34 and 4 cases with high expression of miR-194;Low expression was found in gradeⅣ.Through a 3-year follow-up,we learned that the survival rate of patients in the miR-34 high expression group was higher than that in the low expression group(P<0.05);The survival rate of miR-194 high expression group was higher than that of low expression group(P<0.05).According to the ROC curve,the AUC of miR-34 and miR-194 in predicting the survival period of patients were 0.809 and 0.764,respectively,and the cutoff values were 2.11 and 2.05,respectively.The AUC predicted by the c
