胃癌患者血清IL-17表达及其与微血管形成的相关性分析

摘　　要：目的探讨胃癌患者血清白介素-17(IL-17)表达及其与微血管形成的相关性。方法选取胃癌患者56例作为观察组,另选同期健康体检者48例作为对照组。所有研究对象均清晨采集空腹血,检测IL-17、血管内皮生长因子(VEGF)水平,并分析胃癌患者血清IL-17水平与年龄、性别、TNM分期、淋巴结转移等临床病理特征的关系;绘制ROC曲线分析血清IL-17、VEGF在胃癌中的诊断价值;采用Pearson相关系数分析IL-17与VEGF的相关性。结果观察组血清IL-17[(63.19±5.14)pg/mL]、VEGF[(246.89±12.87)pg/mL]水平高于对照组[(33.12±3.28)pg/mL、(58.76±5.36)pg/mL],差异有统计学意义(P<0.05);有淋巴结转移、TNM分期Ⅲ~Ⅳ期、进展期胃癌患者血清IL-17水平高于无淋巴结转移、TNM分期Ⅰ~Ⅱ期、早期胃癌患者,差异有统计学意义(P<0.05);绘制ROC曲线分析显示,IL-17水平、VEGF水平诊断胃癌的曲线下面积分别为0.702、0.688,IL-17诊断效能良好,VEGF诊断效能一般;经Pearson相关系数分析显示,IL-17水平与VEGF水平呈明显正相关(γ=0.628,P<0.05)。结论胃癌患者血清内IL-17表达远高于健康人,其表达与淋巴结转移、TNM分期存在密切关系,早期检测IL-17能够辅助胃癌诊断,且IL-17与微血管形成存在正相关性,或可通过VEGF参与胃癌进展及转移。Objective To investigate the expression of serum interleukin-17(IL-17)in patients with gastric cancer and its correlation with angiogenesis,so as to provide reference for clinical understanding of tumor progression and metastasis mechanism.Methods 56 patients with gastric cancer treated were selected as the observation group,and 48 healthy people who underwent physical examination in our hospital in the same period were selected as the control group.All subjects collected fasting blood in the morning to detect the levels of IL-17 and vascular endothelial growth factor(VEGF).The differences of various indexes between the 2 groups were compared,and the relationship between the level of serum IL-17 and clinicopathological features such as age,gender,TNM stage and lymph node metastasis in patients with gastric cancer was analyzed;Draw ROC curve to analyze the diagnostic value of serum IL-17 and VEGF in gastric cancer;Pearson correlation coefficient was used to analyze the correlation between IL-17 and VEGF.Results the levels of serum IL-17[(63.19±5.14)pg/ml]and VEGF[(246.89±12.87)pg/ml]in the observation group were significantly higher than those in the control group[(33.12±3.28)pg/ml and(58.76±5.36)pg/ml](P<0.05);The level of serum IL-17 in patients with lymph node metastasis,TNM stageⅢ~Ⅳand advanced gastric cancer was significantly higher than that in patients without lymph node metastasis,TNM stageⅠ~Ⅱand early gastric cancer(P<0.05);The ROC curve analysis showed that the areas under the curve of IL-17 level and VEGF level in the diagnosis of gastric cancer were 0.702 and 0.688 respectively.The diagnostic efficacy of IL-17 was good and that of VEGF was general;Pearson correlation coefficient analysis showed that the level of IL-17 was significantly positively correlated with the level of VEGF(γ=0.628,P<0.05).Conclusion The expression of IL-17 in serum of patients with gastric cancer is much higher than that of healthy people.Its expression is closely related to lymph node metastasis and TNM stage.Early
