3.0T MRI读出方向上的分段扩散成像的ADC值对老年直肠癌固有肌层突破的诊断价值

摘　　要：目的分析3.0T MRI读出方向上的分段扩散成像的ADC值对老年直肠癌固有肌层突破的诊断价值。方法选择23例老年直肠癌患者作为研究对象,对患者行3.0T MRI分段扩散成像检查并计算ADC值,按照病灶是否突破固有肌层分为未突破组和突破组。利用spearman相关性检验分析ADC值与直肠癌固有肌层突破的关系,绘制ROC曲线分析ADC值预测老年直肠癌固有肌层突破的价值。结果TNM分期T1~T2期患者ADC值明显高于T3~T4期,且差异存在统计学意义(P<0.05);老年直肠癌固有肌层未突破组患者ADC值明显高于突破组,且差异存在统计学意义(P<0.05)。3.0T MRI读出方向上的分段扩散成像的ADC值与老年直肠癌固有肌层突破呈明显负相关关系,且存在统计学意义(P<0.05);3.0T MRI读出方向上的分段扩散成像的ADC值预测老年直肠癌固有肌层突破的AUC>0.85,具有较高的应用价值。结论3.0T MRI读出方向上的分段扩散成像的ADC值与老年直肠癌固有肌层突破密切相关,且具有较高的预测老年直肠癌固有肌层突破的诊断价值。Objective To analyze the value of ADC value of segmented diffusion imaging in the readout direction of 3.0T MRI in the diagnosis of intrinsic muscularis breakthrough in elderly rectal cancer,and to provide basis for clinical application.Methods 23 elderly patients with rectal cancer were selected as the research object.According to the rectal adenocarcinoma confirmed by postoperative pathology,the patients were divided into non breakthrough group and breakthrough group according to whether the lesion broke through the muscularis propria;The patients were examined by 3.0T MRI and segmented diffusion imaging,and the ADC value was calculated;Spearman correlation test was used to analyze the relationship between ADC value and intrinsic myometrial breakthrough of rectal cancer,draw ROC curve,and analyze the value of ADC value in predicting intrinsic myometrial breakthrough of elderly rectal cancer.Results The ADC of patients with TNM stage T1~T2 was significantly higher than that of patients with TNM stage T3~T4(P<0.05);The ADC value of elderly patients with rectal cancer in the non breakthrough group was significantly higher than that in the breakthrough group,and the difference was statistically significant(P<0.05);The ADC value of segmented diffusion imaging in the readout direction of 3.0T MRI was significantly negatively correlated with the breakthrough of proper muscle layer of elderly rectal cancer(P<0.05);The ADC value of segmented diffusion imaging in the readout direction of 3.0T MRI was more than 0.85,which had high application value.Conclusion the ADC value of segmented diffusion imaging in the readout direction of 3.0T MRI is closely related to the intrinsic myometrial breakthrough of elderly rectal cancer,and has high diagnostic value in predicting the intrinsic myometrial breakthrough of elderly rectal cancer.
