摘 要:目的分析食管癌根治术后伴肺部感染人群菌群分布情况及影响肺部感染发生的因素。方法将收治的符合标准的82例食管癌患者依照是否出现肺部感染分为感染组和非感染组;患者均接受食管癌根治术和术后辅助放疗,治疗结束后采集患者肺部痰液标本,并对患者菌株进行鉴定;采用logistics回归模型分析影响肺部感染发生因素。结果共分离出26株菌株,其中革兰氏阴性菌感染共分离出17株,占比最高,且差异存统计学意义(P<0.05);感染组与非感染组患者年龄、BMI、吸烟、肺转移、肿瘤位置、合并症间存明显差异,且有统计学意义(P<0.05);年龄、BMI、吸烟、肺转移、肿瘤位置及合并症是影响食管癌根治术后伴肺部感染人群并发肺部感染的独立影响因素(P<0.05)。结论食管癌根治术后伴肺部感染人群以革兰氏阴性菌为主,且年龄、BMI、吸烟、肺转移、肿瘤位置及合并症是影响食管癌根治术后伴肺部感染人群病发肺部感染的独立性影响因素。Objective To analyze the distribution of bacteria in patients with pulmonary infection after radical resection of esophageal cancer and the influencing factors of pulmonary infection.Methods 82 patients with esophageal cancer were divided into infection group and non infection group according to whether they had pulmonary infection.All patients were treated with radical resection of esophageal cancer and postoperative adjuvant radiotherapy.After treatment,sputum samples were collected and identified.Logistic regression model was used to analyze the impact of pulmonary infection.There were many factors affecting the occurrence of the disease.Results A total of 26 strains were isolated,including 17 strains of Gram-negative bacteria infection,accounting for the highest proportion,and the difference was statistically significant(P<0.05);there were significant differences in age,BMI,smoking,lung metastasis,tumor location and complications between the infection group and the non infection group,and the difference was statistically significant(P<0.05);there were significant differences in age,BMI,smoking,lung metastasis,tumor location and complications between the 2 groups,and the difference was statistically significant(P<0.05)The incidence of pulmonary infection in patients with pulmonary infection after radical esophagectomy was significantly higher than that in patients with pulmonary infection after radical esophagectomy(P<0.05).Conclusion Gram negative bacteria are the main pathogens in patients with pulmonary infection after radical esophagectomy.Age,BMI,smoking,lung metastasis,tumor location and complications are the independent influencing factors of pulmonary infection in patients with pulmonary infection after radical esophagectomy.
