机器人辅助宫颈癌根治术后并发症发生的危险因素研究

摘　　要：目的研究机器人辅助宫颈癌根治术后并发症发生的危险因素。方法选择郑州大学第一附属医院治疗的125例宫颈癌患者,均行机器人辅助宫颈癌根治术治疗。收集患者一般资料、病理资料、临床血液指标及手术指标。术后随访6个月,依据并发症发生与否将患者分为有并发症组、无并发症组,分析影响术后并发症发生的危险因素。结果125例宫颈癌患者术后发生并发症18例,并发症发生率14.40%(18/125)。有并发症组合并症发生率及年龄、体重指数(BMI)、C反应蛋白(CRP)水平高于无并发症组,有统计学差异(P<0.05);2组的肿瘤分期、病理分型、盆/腹腔手术史、肿瘤直径、手术时间、术中出血量及CA125比较,无统计学差异(P>0.05)。Logistic回归分析显示,合并症、年龄、BMI及CRP为术后并发症发生的独立危险因素(P<0.05且OR≥1)。结论机器人辅助宫颈癌根治术后并发症的独立危险因素主要有合并症、年龄、BMI及CRP。针对存在上述高危因素的患者,临床需要采取积极的预防措施,以减少并发症的发生。Objective To study the risk factors for complications after robotic assisted cervical cancer.Methods 125 cervical cancer patients were treated with robot assisted radical cervical cancer.General patient data,pathological data,clinical blood indicators and surgical indicators were collected.After the postoperative follow-up of 6 months,patients were divided into complication and non-complication groups according to complications or not.Statistical complications and analyzed risk factors affecting postoperative complications.Results 125 cervical cancer patients had 18 postoperative complications,Complications incidence of 14.40%(18/125);The incidence of complications and age,body mass index(BMI),and C response protein(CRP)levels were higher than in the non-complication group,There were statistical differences(P<0.05);Comparison of tumor stage,pathological typing,history of pelvic/abdominal surgery,tumor diameter,surgical time,intraoperative bleeding and CA125 between the 2 groups had no statistical difference(P>0.05);The Logistic regression analysis showed that,comorbidities,age,BMI,and CRP were independent risk factors affecting postoperative complications(P<0.05 and OR≥1).Conclusion The independent risk factors for complications of cervical cancer are mainly comordities,age,BMI and CRP,for patients with the above risk factors,and active preventive measures are needed to reduce the occurrence of complications.
