摘　　要：目的分析乳腺癌术后Ⅱ期假体植入围手术期感染的影响因素。方法选取58例乳腺癌术后Ⅱ期假体植入围手术期感染患者作为发生组,并以1∶1配比选取同期乳腺癌术后Ⅱ期假体植入围手术期未发生感染患者58例作为未发生组。采用我院自制调查问卷收集两组术中出血量、是否存在糖尿病、是否保留乳头乳晕、TNM分期等信息,分析乳腺癌术后Ⅱ期假体植入围手术期感染的影响因素。结果年龄、Ⅱ期植入手术时间、体质指数、是否行腋下淋巴结清扫,是影响乳腺癌术后Ⅱ期假体植入围手术期感染的单因素(P<0.05);logistic回归分析显示,年龄>45岁、体质指数≥28 kg/m^(2)、行腋下淋巴结清扫是乳腺癌术后Ⅱ期假体植入围手术期感染的危险因素,Ⅱ期植入手术时间≤300 min是乳腺癌术后Ⅱ期假体植入围手术期感染的保护因素(P<0.05)。结论乳腺癌术后Ⅱ期假体植入围手术期感染的影响因素复杂,应根据相关危险及保护因素,加强围术期感染监测,及时行术后预防性感染治疗,降低感染风险,提高乳房再造成功率。Objective To analyze the factors of perioperative infection with postoperative prosthesis implant in breast cancer.Methods 58 patients with breast cancer perioperative infection with postoperative prosthesis implant and 58 patients not infected were selected.The amount of intraoperative bleeding,the presence of diabetes,the papillary areola,TNM stage and other information,were used to analyze the influencing factors of perioperative prosthesis infection in breast cancer.Results Age,implantation,constitution index,and axillary lymph node dissection were single factors influencing perioperative prosthetic infection in breast cancer(P<0.05);logistic regression analysis showed that>45 and≥28 kg/m^(2),was a risk factor for perioperative infection in breast cancer,and≤300 min was a protective factor for perioperative infection in breast cancer(P<0.05).Conclusion The influencing factors of perioperative prosthesis infection in breast cancer are complex.According to the relevant risk and protective factors,strengthen the monitoring,conduct preventive treatment of infection,reduce the risk of infection and improve the success rate of breast reconstruction.
