摘　　要：目的比较肩胛舌骨上颈淋巴结清扫术(SOND)与功能性颈淋巴结清扫术(FND)对cN0/N1期口腔鳞癌患者的效果。方法选取94例cN0/N1期口腔鳞癌患者,按随机数字表法分为两组,各47例。对照组行FND,观察组行SOND,观察至术后1年。对比两组疾病复发率、生存质量、并发症。结果两组复发率相当,差异无统计学意义(P>0.05);观察组美国华盛顿大学生存质量问卷(UW-QOL)内各项评分(面容外形、活动能力、消遣娱乐、工作、咀嚼、语言、吞咽、肩部功能、疼痛)均高于对照组,并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论FND与SOND治疗cN0/N1期口腔鳞癌效果相当,可减少疾病复发,提高短期生存率,但SOND后并发症更少,有利于提高患者生存质量,安全可行,值得应用。Objective To compare the effect of suprahyoid neck dissection(SOND)and functional neck dissection(FND)on cN0/N1 oral squamous cell carcinoma.Methods 94 patients with cN0/N1 oral squamous cell carcinoma were randomly divided into 2 groups,47 patients in each group.FND was performed in the control group and SOND was performed in the observation group until 1 year after operation.The recurrence rate,quality of life and complications of the 2 groups were compared.Results The recurrence rates of the 2 groups were similar,with no significant difference(P>0.05);The scores(facial appearance,activity ability,recreation,work,chewing,language,swallowing,shoulder function,pain)in the UW-QOL in the observation group were higher than those in the control group,and the incidence of complications was lower than that in the control group,with statistical significance(P<0.05).Conclusion FND is as effective as SOND in the treatment of cNO/N1 oral squamous cell carcinoma,which can reduce disease recurrence and improve short-term survival rate.However,there are fewer complications after SOND,which is beneficial to improve the quality of life of patients,safe and feasible,and worthy of application.
