摘 要:目的 探讨外周血循环肿瘤细胞(CTCs)联合血清肿瘤标志物检测在非小细胞肺癌(NSCLC)中的临床意义。方法 选取2020年1月至2022年1月我院收治的NSCLC患者102例作为研究组,同期健康体检者87例作为对照组。比较研究组与对照组、不同临床分期患者外周血CTCs、血清肿瘤标志物的表达情况,分析影响预后的因素及诊断价值。结果 研究组及Ⅲ~Ⅳ期患者CTCs阳性表达癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、细胞角蛋白19血清片段21-1(CYFRA21-1)水平均高于对照组及Ⅰ~Ⅱ期患者(P<0.05);年龄、临床分期、CTCs、CEA、NSE、CYFRA21-1为患者预后的影响因素(P<0.05);外周血CTCs联合血清肿瘤标志物检测灵敏度、特异度、AUC最高(P<0.05)。结论 外周血CTCs联合血清肿瘤标志物检测可有效诊断NSCLC,可为临床病情诊断及预后评估提供参考。Objective To investigate the clinical significance of peripheral blood circulating tumor cells(CTCs) combined with serum tumor markers in patients with non-small cell lung cancer(NSCLC).Methods One hundred and two NSCLC patients from January 2020 to January 2022 in our hospital were selected as a research group, and 87 healthy individuals who underwent physical examination at the same time were selected as a control group. The peripheral blood CTCs and serum tumor markers were compared between the two groups and among the patients with different clinical stages. The prognostic factors and diagnostic value were analyzed.Results CTCs positive rate and levels of carcinoembryonic antigen(CEA), neuron-specific enolase(NSE) and cytokeratin 19 serum fragment 21-1(CYFRA21-1) in the research group and patients with stage III-IV were higher than those in the control group and patients with stage I-II(P<0.05). Age, clinical stage, CTCs, CEA, NSE and CYFRA21-1 were the influencing factors of the patient′s prognosis(P<0.05). The detection sensitivity, specificity and area under ROC curve(AUC) of peripheral blood CTCs combined with serum tumor markers were the highest.Conclusions The detection of peripheral blood CTCs combined with serum tumor markers can effectively diagnose NSCLC. It can also provide reference for clinical diagnosis and prognosis evaluation.
