早期肠内免疫营养支持对重症脑卒中患者营养状况及免疫功能的影响

摘　　要：目的探究重症脑卒中患者进行早期肠内免疫营养支持对营养状况及免疫功能的影响。方法选择82例重症脑卒中患者作为独立样本进入本次研究,收治时间均为2019年5月至2022年5月,所有病例根据计算机抽签法分组,两个研究小组中的试验组(41例)进行早期肠内免疫营养支持,而参照组(41例)进行常规的鼻饲管喂食,对比不同组别患者之间的营养指标、免疫功能指标变化及并发症情况。结果试验组患者第15天的血红蛋白(Hb)、前白蛋白(PA)、白蛋白(Alb)、血清总蛋白(TP)远高于参照组,统计学意义成立(P<0.05)。试验组患者第15天的IgA、IgM、IgG以及CD8^(+)、CD4^(+)、CD4^(+)/CD8^(+)均远高于参照组,统计学意义成立(P<0.05)。试验组患者并发症发生率远低于参照组,分别为41.46%和70.73%,统计学意义成立(P<0.05)。结论重症脑卒中患者接受早期肠内免疫营养支持,能够有效改善营养状况、提高免疫功能、减少并发症,具有推广应用价值。Objective To explore the effect of early enteral immunonutrition support on nutritional status and immune function in patients with severe stroke.Methods Choose 82 cases of severe stroke patients into the study as an independent sample,treated time are all from May 2019 to May 2022,All cases were grouped according to computer lottery.The experimental group(41 cases)with early enteral immune nutritional support,and the control group(41 cases)for routine nasogastric tube feeding.The changes of nutritional indexes,immune function indexes and complications were compared between different groups of patients.Results The hemoglobin(Hb),prealbumin(PA),albumin(Alb)and serum total protein(TP)of the experimental group were significantly higher than those of the control group on the 15th day,and the statistical significance was established(P<0.05).The levels of IgA,IgM,IgG,CD8^(+),CD4^(+),CD4^(+)/CD8^(+)in the experimental group on the 15th day were significantly higher than those in the control group(P<0.05).The incidence of complications in the experimental group was significantly lower than that in the control group,41.46%and 70.73%,respectively,with statistical significance(P<0.05).Conclusions Early enteral immunonutrition support for patients with severe stroke can effectively improve nutritional status,improve immune function and reduce complications,which has the value of popularization and application.
