湖北某地区上消化道出血的临床流行病学分析

摘　　要：目的:研究上消化道出血患者的病因及相关临床特点。方法:选取2016年1月-2018年12月[1]湖北黄石地区二级及三级医院消化内科收治的上消化道出血患者3 649例,对患者的年龄、性别、入院时间、住院天数、病因、合并的慢性疾病、是否服用非甾体抗炎药、吸烟、饮酒、转归等进行统计学分析。结果:上消化道出血患者平均年龄(53.08±16.84)岁,常见病因为十二指肠溃疡、急性胃黏膜病变、胃溃疡、肝硬化食管胃底静脉曲张、胃癌。年龄越大,平均住院时间越长,死亡率越高。结论:上消化道出血多见于中老年男性,消化性溃疡、急性胃黏膜病变、肝硬化食管胃底静脉曲张、胃癌为主要病因,与吸烟、饮酒等不良生活习惯相关。Objective: To study the etiology and related clinical features of upper gastrointestinal bleeding patients. Methods:From January 2016 to December 2018[1], 3649 patients with upper gastrointestinal bleeding who were hospitalized ingastroenterology department of secondary and tertiary hospitals in Huangshi of Hubei Province were selected. The patient’s age,gender, admission time, length of hospital stay, etiology, combined chronic diseases, presence or absence of nonsteroidal anti-inflammatory drugs, smoking, alcohol drinking, and prognosis were statistically analyzed. Results: The average age of patients withupper gastrointestinal bleeding was(53.08±16.84) years old. The common causes were duodenal ulcer, acute gastric mucosaldisease, gastric ulcer, esophageal and gastric varices in liver cirrhosis, and gastric cancer. The older the patients were, the longerthe average length of their hospital stay was, and the higher the mortality rate was. Conclusion: Upper gastrointestinal bleeding ismore common in middle-aged and elderly men. The main causes are peptic ulcer, acute gastric mucosal disease, esophageal andgastric varices in liver cirrhosis and gastric cancer. It is related to smoking, alcohol drinking and other bad living habits.
