MUC4、CUEDC2、miR-193a-5p在子宫内膜癌组织中的表达及其临床价值

摘　　要：目的 探讨子宫内膜癌组织中黏蛋白4(MUC4)、CUE结构域蛋白2(CUEDC2)、微小RNA-193a-5p(miR-193a-5p)的表达情况及其临床价值。方法 选取2014年5月至2016年5月该院接受手术治疗的84例子宫内膜癌患者及84例子宫内膜息肉患者的病历资料进行回顾性研究。采用免疫组织化学法检测子宫内膜癌组织、癌旁组织及子宫内膜息肉组织中MUC4、CUEDC2表达情况,采用聚合酶链反应检测这3种组织中miR-193a-5p表达水平。比较子宫内膜癌组织、癌旁组织及子宫内膜息肉组织MUC4、CUEDC2、miR-193a-5p表达情况,分析不同病理特征子宫内膜癌组织MUC4、CUEDC2、miR-193a-5p表达情况,采用Kaplan-Meier生存曲线分析不同MUC4、CUEDC2、miR-193a-5表达情况子宫内膜癌患者5年生存率,采用Cox回归模型分析子宫内膜癌患者预后的影响因素。结果 子宫内膜癌组织MUC4、CUEDC2阳性表达率高于癌旁组织、子宫内膜息肉组织(P<0.05),miR-193a-5p表达水平低于癌旁组织、子宫内膜息肉组织(P<0.05)。与肌层浸润深度<1/2、FIGO分期Ⅰ~Ⅱ期、无淋巴结转移的子宫内膜癌组织相比,肌层浸润深度≥1/2、FIGO分期Ⅲ期、有淋巴结转移的子宫内膜癌组织MUC4、CUEDC2阳性表达率更高(P<0.05),miR-193a-5p表达水平更低(P<0.05)。子宫内膜癌组织MUC4、CUEDC2阳性患者5年生存率低于阴性患者(P<0.05),miR-193a-5p低表达患者5年生存率低于高表达患者(P<0.05)。Cox回归模型分析显示,阳性的MUC4、CUEDC2及低表达的miR-193a-5p是子宫内膜癌患者5年内死亡的独立危险因素(HR=3.582、4.171、0.390,P<0.05)。结论 子宫内膜癌组织中MUC4、CUEDC2高表达,miR-193a-5p低表达,它们与肿瘤肌层浸润深度、FIGO分期、淋巴结转移有关,且为影响患者预后的独立危险因素。Objective To investigate the expression and clinical value of mucin 4(MUC4),CUE domain containing 2(CUEDC2) and microRNA-193 a-5 p(miR-193 a-5 p) in endometrial carcinoma tissues.Methods The medical records of 84 patients with endometrial carcinoma and 84 patients with endometrial polyps who received surgical treatment in this hospital from May 2014 to May 2016 were selected for a retrospective study.The expression of MUC4 and CUEDC2 in endometrial carcinoma tissues, paracancerous tissue and endometrial polyp tissues was detected by immunohistochemistry, and the expression level of miR-193 a-5 p in the three tissues was detected by polymerase chain reaction.The expression of MUC4,CUEDC2 and miR-193 a-5 p in endometrial carcinoma tissues, paracancerous tissues and endometrial polyp tissues was compared.The expression of MUC4,CUEDC2 and miR-193 a-5 p in endometrial carcinoma tissues with different pathological features was analyzed.Kaplan-Meier survival curve was used to analyze the 5-year survival rate of endometrial carcinoma patients with different expressions of MUC4,CUEDC2 and miR-193 a-5,and Cox regression model was used to analyze the prognostic factors of endometrial carcinoma patients.Results The positive expression rates of MUC4 and CUEDC2 in endometrial carcinoma tissues were higher than those in paracancerous tissue and endometrial polyp tissues, and the expression level of miR-193 a-5 p in endometrial carcinoma tissues was lower than that in paracancerous tissue and endometrial polyp tissues(P<0.05).Compared with endometrial carcinoma tissues with myometrial invasion depth <1/2,FIGO stage Ⅰ-Ⅱ and no lymph node metastasis, the endometrial carcinoma tissues with myometrial invasion depth ≥1/2,FIGO stage III and lymph node metastasis had higher positive expression rates of MUC4 and CUEDC2(P<0.05),and lower expression level of miR-193 a-5 p(P<0.05).The 5-year survival rate of patients with positive MUC4 and CUEDC2 in endometrial carcinoma tissues was lower than that of patients with negative MUC4 and
